fix(ai-cohere): split embed() input into batches of at most 96 - #1984
Root cause.CohereEmbeddingAdapter.createEmbeddings builds one inputs array for the whole call and sends it in a single POST /v2/embed. Cohere accepts at most 96 inputs per request, so embed() with more than 96 texts fails with a 400 from the API. The sibling Bedrock adapter already handles this for Cohere models with COHERE_MAX_BATCH_SIZE = 96; this adapter did not.
Fix.createEmbeddings now sends the inputs in requests of at most 96 (COHERE_MAX_BATCH_SIZE), one after the other. Vectors are appended in input order and re-indexed from 0, inputTokens from meta.billed_units are summed, and the existing per-request checks (float embeddings present, count matches) run per batch. The request body for 96 or fewer inputs is unchanged (a single request, same fields). Only packages/ai-cohere/src/adapters/embedding.ts changes, plus a patch changeset.
Possible alternatives. Send the batches concurrently (like Bedrock's fan-out): faster for large inputs, but it adds a concurrency knob and rate-limit exposure that this fix does not need. Or throw a clear error above 96 and leave chunking to callers: that moves the same loop into every caller and embed() takes arbitrary arrays.
Test.packages/ai-cohere/tests/embedding-adapter.test.ts gets one test: 200 texts, with a mocked endpoint that answers 400 for any request over 96 inputs, like Cohere does. It expects three requests of 96, 96 and 8, 200 vectors in order with indexes 0 to 199, and summed usage. It fails on main with the 400 and passes with this change; the other 30 tests in the package still pass, tsc and oxfmt --check are clean. No live API calls were made.
Docs. Not changed: docs/adapters/cohere.md does not describe how embedding requests are batched, so nothing there becomes wrong.
Cohere's v2/embed rejects requests with more than 96 inputs. Send the input
in sequential requests of at most 96, return the vectors in input order and
sum the billed input tokens.

Walkthrough
The Cohere embedding adapter now sends inputs in batches of up to 96. It validates each response, combines vectors in input order, and sums available input-token counts across batches.

The adapter sends batches of up to 96 inputs and validates each response against its batch size. It appends vectors in input order and sums available input-token counts. The test checks request sizes of 96, 96, and 8 for 200 inputs, as well as vector order and token totals.

Batching supports larger embedding requests, but very large calls can fail after completing earlier batches, and missing token counts can understate usage. These bounded limitations warrant owner awareness or follow-up.

Make the batching usage test detect a wrong token total.
The mock returns inputs.length for each batch. The expected total of 200 therefore equals the total input count as well as the sum of billed units. A regression that counts inputs instead of summing billed units would still pass. Returning a different count per batch makes the two totals differ.

Update the mock in the batching usage test so each batch reports billed units
that differ from its input count, then adjust the expected promptTokens and
totalTokens to the sum of those billed units; keep completionTokens at zero.
This ensures the test detects input-counting in place of billed-unit
aggregation.
Retry rate-limited embed batches, or pace the loop.
A call with more than 2,000 inputs sends back-to-back batches with no pacing or retry. Cohere's Embed endpoint allows 2,000 inputs per minute and returns 429 when that limit is exceeded. The first batches are billed, then the call throws and discards their vectors. On 429, wait for the Retry-After interval and retry the failed batch, or pace batches within the limit.

Set usage only when every batch reports input tokens.
When one batch omits billed_units.input_tokens, the adapter still returns a usage total summed from the other batches. Consumers record that partial count as the prompt tokens for all inputs, so usage is understated. Keep the total undefined if any batch omits the count.

Thanks for the fix! I checked that the batching loop keeps input order and re-indexes from 0, that the per-batch count check and token summing are correct, and that requests of 96 or fewer are unchanged. The Bedrock adapter already uses the same 96 limit.
