Recovery banners accumulated whenever a source fault cleared, so dismissing one exposed another identical “Source issue no longer reported” notice.

This coalesces earlier recoveries when the same fault recurs, automatically hides recovery banners after ten seconds, and names the affected client and issue. Recent updates retains recovery history; current faults and evidence-based resolution rules are preserved.

Validation:

  • 4,901 Python tests passed on CPython 3.11.15.
  • Swift suite: 676 tests, 7 environment-gated skips, 0 failures. Regression coverage includes recurring faults, exact expiry, independent deadlines, manual dismissal, preserved active faults/history, and a mounted-window timer with no health refresh.
  • Independent code/design review and whitespace checks passed.

The 0.12.10 version bump and rebuilt signed/notarized DMG follow in the separate release PR after this fix merges.
