fix(ai-harness): store a rejected input with its rejection - #1955
The session resolved a rejected receipt before the log stored harness.input.rejected. A crash in that gap ran the input again. This PR writes the input and the rejection in one append. The receipt resolves after the log has that record.
admit() now decides a refusal before it writes. One append stores harness.input and harness.input.rejected. The rejected receipt waits for that append. A failed write fails the operation, and recovery does not run the input.
I have tested code changes locally with pnpm run test:pr, or these tests do not apply to this pull request.
I fully understand the code in this pull request, including any code generated with AI assistance.
Docs: I updated docs/ for this change, or this change is not user-facing.
Changeset: I added a changeset (pnpm changeset), or this PR does not change a published package.
🚀 Release Impact
This change affects published code, and I have generated a changeset.
This change is docs/CI/dev-only (no release).
Root cause
Issue. A caller with a rejected receipt can read an input that has no outcome. After a crash, the next host runs that input.
Cause.admit() stores harness.input. reject() then appends harness.input.rejected and does not wait. The receipt returns between the two writes.
Fix.rejectIf decides the refusal before the append. The same append stores harness.input and harness.input.rejected.
Possible alternatives
Wait for the second append. The receipt then waits, and a failed write is visible. A crash between the two appends still leaves a pending input. This PR does not take that path.
Testing

On main, hold the append that contains harness.input.rejected.
When a client tool interrupt is open, send a follow-up.
If that append is still held, make sure that the receipt is already rejected.
On this branch, repeat steps 1 and 2.
Make sure that the receipt stays pending until the append lands. The batch has harness.input and harness.input.rejected.
How this PR makes testing easy.packages/ai-harness/tests/rejection-durability.test.ts holds the rejection append, crashes the owner, fails the write, and checks each refusal reason.
Risk / rollback
Revert the PR. A prompt, a tool, a reset, or a resolve that the session refuses now lands in the admission append. A failed write fails the operation. The input does not stay pending.
Summary by CodeRabbit
New Features
Rejected inputs are recorded alongside their rejection outcome. Receipts resolve after the outcome is stored, and recovery won’t run an input that was rejected.
Repeated submissions return the recorded rejection when the payload matches; a different payload is reported as a conflict.
Session refusals are handled before inputs are stored, including refusals involving busy sessions, pending interrupts, and unresolved questions.
Documentation
Updated the harness guides to explain rejection receipts and recovery behavior.
The rejected receipt resolved before harness.input.rejected was in the log, so a crash could run that input. The refusal is decided before the append, and both records go in that one append.

Walkthrough
The harness now checks specified refusal conditions before storage, appends rejected inputs and rejection records together, and resolves rejected receipts after the append. Tests cover durability, replay, conflicts, and recovery behavior.
Admission appends a rejected input and its rejection record together. Rejected receipts resolve after the append. Tests cover append failures, recovery, replay, and payload conflicts. Documentation describes the rejection and recovery behavior.
Turn, tool, reset, and resume paths check refusal conditions before storage. Resume handling claims eligible background waits and releases claims in duplicate, completion, and error paths. Tests verify atomic records for refusal cases.
This change makes rejection of the main turn, tool, reset and resume inputs crash-safe. Rejections from cancel, cancelInput, setDelivery and the resolve fallback paths can still be lost on a write failure, because their receipts resolve before the rejection is stored. Under concurrent submissions, a prompt sent with busy: 'reject' may be queued instead of refused. Resolve or explicitly accept these gaps before merging.

Await the rejection write and update all reject() callers.
reject() currently suppresses append failures and publishes the receipt before harness.input.rejected is durable. The request methods already return this.reject(...), so they will propagate an async rejection. Other callers must await it. In particular, operation.resolveReceipt(...) accepts a Receipt, not a promise, and callers that ignore reject() still would not surface write failures.

Keep the refusal decision consistent with turn scheduling.
admitting() only increments inputsInFlight; it does not serialize admissions. refusalOfTurn() runs before await this.writer.append(...), so another admission can start a turn or leave interrupted set before this input is enqueued.
A busy: 'reject' prompt can therefore pass while idle, then queue behind a turn admitted concurrently. A follow-up can pass before a turn stops with interrupts, then queue instead of receiving pending_interrupts. This is a narrow timing window, but it changes the documented refusal behavior.
Synchronize the refusal decision with the state transition that enqueues the turn, or re-evaluate the state and durably reject the stored input when the state changes. A post-append check must preserve the crash-safe input/rejection recording contract.
