feat(eve): expose session to approval predicates

What

• Give custom needsApproval predicates the session metadata already available to execute.
• Cover normal tools, code-mode host tools, dynamic tools, and approval-resume with a real context scope.
• Add a schedule fixture and document the per-turn auth rule.

Why this shape

Schedule task roots cannot wait for an approval response. A tool can skip approval for the framework's full schedule principal and still require it when a person resumes the same session. Matching only principalId is too weak, so the example checks the authenticator, id, and type.
