README was missing npm and CI status badges in the header.

Changes

  • npm version badge — links to utoo on npmjs.com, shows current published version
  • npm downloads badge — monthly download count for utoo
  • PM CI badge — live status for pm-ci.yml workflow
  • Pack CI badge — live status for pack-ci.yml workflow

All four badges are appended to the existing badge row in the <div align="center"> header block, consistent with the existing style=flat-square convention.
