feat(doc-approval): Spec 001/002 event-UI parity across shells

Summary

• Apply Specs 001/002 event→presentation mapping across doc-approval shells (web, Rust CLI/GTK, Swift, Android, Windows).
• Add PresentationMapper ports + HostRunResult presentation fields; surface state in HealthIndicator / native chrome.
• Unit tests for web host, Rust core, and native mapper fixtures.

• CI: repository-checks, quality, coverage-gate, pr-hygiene, embedded-smoke/native-linux as applicable
