docs(readme): clarify stack profiles & overrides are host-agnostic - #78

The "Stack-agnostic by design" section implied only CLAUDE.md overrides the shipped stack profiles. The profiles travel with the plugin and work on every host, and the override is whatever instructions file the host agent loads: CLAUDE.md on Claude, AGENTS.md on Codex, .github/copilot-instructions.md on Copilot.
