fix: detach completion callbacks on reload

Summary

• invalidate extension-owned callbacks before session shutdown cleanup
• detach child listeners while leaving detached jobs and logs running
• reconcile terminal log evidence in the active replacement generation
• prevent headless sessions from skipping reconciliation
