docs: match the schema 26 open path and close finding 085 - #966

The operator upgrade table still described a rewrite that stopped at schema 25 and grew headers. Open shrinks 24/25 headers to 88 bytes and stamps meta 26. The IBD mutated-body path for finding 085 is already fixed. Co-authored-by: Cursor <cursoragent@cursor.com>
