Adds the first real implementation build plan for OSOH monitored-surface support inside the Astro/Node Sentinel repository.

Includes:

minimal server-side store layer plan
token resolution and validation plan
first ingest route plan
inactive-by-default monitored surface plan
external monitored surface classification preservation
ingestion-only scope preservation
fail-closed activation preservation
