Summary

• Refreshes README, DEVELOPMENT, AGENTS, and TEST_COVERAGE project/test inventory so they match the current src/ and tests/ layout.
• Corrects stale setup, release, gateway LKG, A2UI, MCP command, Mission Control chat UX, onboarding, setup, and data-flow documentation.
• Adds docs/ACCESSIBILITY.md for the Axe.Windows CI quality gate and documents the current GitVersion local/CI pin split in docs/VERSIONING.md.

Notes / blockers

• Plan item 1.9 used the documented-split fallback instead of changing .github/workflows/ci.yml; this branch is docs-only because the available push token cannot update workflow files.
• docs/CONNECTION_ARCHITECTURE.md already described PairingApprovalCoordinator on current main, so no duplicate section was added.
