Unify all cases and remove quarantine labeling

Summary

• Publish all 6,089 existing cases in one default configuration, without audit_status.
• Preserve IDs, prompts, answers, provenance and individual issue notes; old revisions remain accessible.
• Add a deterministic hash-checked conversion from the public historical reconstruction.
• Simplify the dataset card and update current-release documentation and revision ledger.

Validation

• 28 tests passed; Hugging Face card validation passed.
• Full case comparison confirms only audit_status was removed.
• CI rebuilds historical inputs and then the unified release.
