Conversation
- Cut the search guidance to one clause; the CLAUDE.md template carries it in full. - Trigger continuous-learning on a found cause, a decision, or before a task's final reply. - Fix a wrong, outdated or incomplete memory immediately instead of at task end.

Why
Sessions skipped continuous-learning at the end of a task, once after a PR printed its URL and once after a refactor left a memory incomplete. The reminder buried capture third, under search guidance that the installed CLAUDE.md section already carries, and the identical shouted block read as background. It now leads with capture and ties it to concrete moments: a found cause, a decision, and the final reply for a task. A wrong, outdated or incomplete memory is fixed immediately instead of waiting for the task to end.
Test plan
mcs sync, then send any prompt → expect the new one-paragraph reminder.
