docs(adr): record ADR-0054's rung-2 dogfood and ADR-0055's implementation evidence

What and why

This records verified evidence in the decision records. It's docs only, apart from a link fix in code comments. No status or ratification field changes.

• ADR-0054:
  • Item 8 is ticked. https://adrkit.dev/decisions/ serves this repo's portal, built by site.yml with uses: ./packages/ci/portal (feat(site): publish this repository's decision portal at adrkit.dev/decisions/ (#294) #310). Verified by curl: html, the module served as text/javascript, and portal-data.json with version "1", 59 records (matching adr lint), and a real commit. The evidence is in the portal: publish this repository's corpus on adrkit.dev as the rung-2 dogfood (ADR-0054 item 8) #294 comment.
  • The evidence rung moves to 2, a maintainer-owned dogfood. It says plainly that this is not an isolated reference repo and doesn't claim rung 3.
• ADR-0055:
  • Items 1–7 and 9 are ticked against feat(server): serve a read-only REST API over the SDK in the OCI image (ADR-0055, #296) #314 and the CI container smoke that ran on main: runs 38095178433 and 38096645097. Item 8 (APIM measurement) stays open.
  • The evidence rung is 1, implemented.
  • Two places where the build differs from the text are noted, not hidden. The portal is at /portal/{corpus}/, with / redirecting there for one corpus. The clause-9 path hint is portal: when the REST server hosts the portal, point the path view at its governing route (ADR-0055 clause 9) #315, in flight.
• ADR-0053 item 9 stays open. The item says the publication record decides it. docs/sdk-surface.md now names the REST server as the long-lived consumer that exercises every handle member, next to the build-time portal, for that record to cite.
• Links: the broken ADR-0055 link (-th.md → -t.md) is fixed in AGENTS.md, README.md, the CHANGELOG, and nine packages/server/src header comments.
