docs: add operations console normative specification

Summary

• Specification-only documentation change; this does not implement the operations console.
• Adds a standalone normative operations-console specification document.
• Adds a brief README pointer to that document.

Verification

• git diff --check --cached passed before commit.
• Commit contains exactly two documentation files: README.md and docs/operations-console.md.
• Issue URLs are confined to the document's non-normative provenance.
