docs: update README with missing commands and custom scopes

What

Update README quick reference to reflect current CLI functionality.

Why

Post-audit of docs vs code (general-cli-audit) identified gaps in the README commands table and rules registry section.

Changes

• README.md — added devw explain to Commands table
• README.md — expanded devw list with granular subtypes (commands, templates, hooks)
• README.md — added custom scopes kind:name pattern description to Rules Registry section
