Workflow Failure

Workflow: Prompt Optimization
Branch: main
Run: https://github.com/githubnext/gh-aw-wizard/actions/runs/37375541447

Warning

Task Could Not Be Completed: The agent reported that the task could not be performed due to an infrastructure or tool failure.

Reasons:

  • Ollama/model infrastructure described as pre-set-up by the workflow was not actually available in this session, so the hill-climbing optimization loop could not run.

Checked for an existing open 'Prompt optimization:' PR first (gh pr list) — none found, so proceeded to optimization.

Found no running Ollama server on 127.0.0.1:11434 (connection refused) despite the task description stating it had been started. The ollama binary existed at /opt/hostedtoolcache/ollama/0.33.2/x64/ollama, so I started ollama serve manually; it came up and responded to /api/tags with an empty model list (no cached GGUF models present in ~/.ollama/models — blobs/manifests dirs were empty).

Attempted ollama pull hf.co/unsloth/SmolLM2-360M-Instruct-GGUF:Q4_K_M to populate the cache myself; it failed with Forbidden — outbound HTTPS to hf.co/huggingface.co is blocked at the squid egress proxy (403 ERR_ACCESS_DENIED), confirmed via direct curl to (hf.co/redacted) and (huggingface.co/redacted)

Additionally, the Ollama server log showed llama-server binary not found during GPU/CPU discovery, meaning even a successfully pulled model could not actually be run for inference in this environment without a separate llama-server build step.

Net effect: neither of the two required proxy models (Qwen2.5-1.5B-Instruct-GGUF and SmolLM2-360M-Instruct-GGUF) could be loaded, so scripts/prompt-optimizer.mjs --evaluate/--score against `(127.0.0.1/redacted) cannot produce measured scores. No repository files were changed; no PR, review, or review comment was created.

This is a structured incompletion signal (report_incomplete), not a real task outcome. Any other safe outputs emitted alongside this signal (e.g., comments) describe the failure state, not a completed review or action.

    Agent job 37533497322 failed.

    Warning

Task Could Not Be Completed: The agent reported that the task could not be performed due to an infrastructure or tool failure.

    Reasons:

    • Required eval infrastructure was not actually present: no Ollama server was running on 127.0.0.1:11434, ~/.ollama/models was empty (0 blobs), and no GGUF files existed anywhere on disk despite the task stating both proxy models were already downloaded and served.

Verified no open PR titled "Prompt optimization:" exists (only unrelated PR Pattern miner: [pattern-miner] Add VEX Statement Generator curated archetype #318 found), so proceeded to the optimization step.

Checked (127.0.0.1/redacted) and /api/tags: connection refused (no server running). The ollama binary exists only at /opt/hostedtoolcache/ollama/0.33.2/x64/ollama (not on PATH); ollama list reported "could not connect to ollama server". Manually started ollama serve: it came up cleanly but reported "total blobs: 0" and no llama-server GPU runtime binary built. Attempted ollama pull hf.co/unsloth/SmolLM2-360M-Instruct-GGUF:Q4_K_M to self-remediate: failed with "Get (hf.co/redacted) Forbidden" via the sandbox's Squid proxy (172.30.0.10:3128), which returns 403 ERR_ACCESS_DENIED for huggingface.co and hf.co. Confirmed with direct curl to both hosts — same 403 from the proxy, so Hugging Face is not reachable from this sandbox to download the GGUF proxies myself.

Searched the whole filesystem (excluding node_modules) for *.gguf, qwen, smollm and any ollama blob/model directories: found none. No eval-model artifacts of any kind are present.

Consequently I could not run node scripts/prompt-optimizer.mjs --evaluate --all-models ... against the required endpoints — every request failed immediately with "request failed" / parsed as none (0% across all scenarios), which is an infrastructure failure, not a real instruction-quality signal, so no hill-climbing iterations were attempted.

Cleaned up: killed the ollama serve process I started for diagnosis, and confirmed git status --short is clean (no repository files were modified; the only incidental change — an auto-added metadata header in .github/skills/optimize-scenario-prompt/SKILL.md from skill loading — was reverted with git checkout --).

No candidate was scored, src/js/slm.js was not touched, and no pull request or review comment was created, per the "if infrastructure/data is missing" fallback in the task instructions.

This is a structured incompletion signal (report_incomplete), not a real task outcome. Any other safe outputs emitted alongside this signal (e.g., comments) describe the failure state, not a completed review or action.
