Problem

The T3 credential-injection decision of 2026-10-20 places each of the ten files of this theme, and the ledger still carries the pre-decision state: no date on the path-prefix rows, the Envoy gRPC-denial row carried although this line runs the agentgateway dataplane, and no fallback on the further-providers and extra-CAs rows whose pull requests have had no review.

Change

Ten headers updated: outcome, exit with its date (2026-11-17 for the path-prefix family, the further providers and the extra CAs) or re-check trigger, refs and dependencies, and one dated History sentence per file carrying the class, the destination, the first step with its target repository, the prepared branch and SHA, the test that proves it, the themes it depends on and the owner. The gRPC-denial row reads to-file, low priority, uncarried at the next re-pin. Documentation only.
