Fall back to an unlabeled transaction when provider fails #5718

If the transaction name provider callback throws, the transaction is dropped. Because a request URL may otherwise remain in the transaction name, this can also risk exposing PII.

• Proposal from Alexander Dinauer: keep the transaction and use <unlabeled transaction> as the name when the callback errors.
