Recent additions concentrated unrelated responsibilities in turn execution, background tasks, provider adapters, and persistence. This restores the existing module boundaries and strengthens import checks so application services cannot depend on transport edges.

  • Separate turn snapshots and telemetry, background scheduling, Codex RPC/schema handling, OAuth credential parsing, and domain-specific database repositories into private modules. Preserve repository exports and transport contracts.
  • Remove the unused LLM execution/retry path, instrumentation helpers, prompt arguments, and their obsolete tests (including the unused session-existence helper); use native message types instead of compatibility probing.
  • Fix mutable in-memory record leakage, rollback after partial startup/cancellation, orphan prompt work on invalid configuration, accumulated usage on promoted steering, memory-only prompt retention, artifact database-error propagation, and Codex RPC subprocess cleanup after read failures.

Validation: Ruff and formatting pass; 2,966 tests pass with one database-dependent skip. All 202 compatibility tests also pass with Pydantic AI 2.40. The voice cleanup regressions also passed with unraisable warnings treated as errors. Database validation runs in CI. Three surveyor reviews completed. Local review used separate CLI processes at high effort: an initial Claude Fable round found four medium issues (all fixed); Astra reviewed the full final diff and found one P2 shutdown race (fixed); Astra's follow-up found no actionable regressions and passed 20 focused shutdown checks. A further Astra high follow-up cleared both CodeRabbit corrections with zero findings. Four completed local review rounds; mis-scoped or interrupted attempts are excluded from the gate. Final reviewed head: ebe3b7d.

Scope: 75 files, +3,200/-3,112 lines, mostly responsibility-preserving moves. No new features or schema changes. README and architecture invariants updated to match.

Part of #83.

Summary by CodeRabbit

  • New Features

    • Background tasks now support capacity limits, ordered execution, and improved cancellation and shutdown handling.
    • Streaming events are delivered through a configurable event sink, with Socket.IO forwarding maintained at the app edge.
    • Codex sign-in can read local CLI credentials and validate compatibility with the realtime protocol.
  • Bug Fixes

    • Prompt snapshots without database storage are isolated by session.
    • In-memory event, task, and action records are protected from unintended changes through shared mutable data.
    • Failed or cancelled startup now rolls back partially started components, even when cleanup is interrupted.
    • Streaming usage and saved assistant responses remain consistent across follow-up turns.

Separate turn snapshots, task scheduling, provider transports and database repositories so each module owns one responsibility. Enforce application layering and remove execution helpers superseded by native Pydantic AI runs.

Preserve validated state and accumulated usage, roll back partially started services, and propagate database failures consistently. Add regressions for the confirmed defects.
Keep memory-only prompt snapshots with their session, drain lifecycle shutdown despite repeated cancellation, and let shared database error handling serve every artifact route. Restrict the settings import exception to the assistant configuration schema.
An oversized response could leave the reader task's exception unobserved and detach the process before its pipes were closed. Drain and reap the failed subprocess before waking dependent calls, and assert complete cleanup in the existing failure regression.
Keep waiter failure and closure delivery unconditional if draining is interrupted, and let normal server shutdown await reader finalization after process exit. Cover shutdown with an outstanding RPC and subscriber.

Layer / File(s) Summary
Prompt-builder inputs
src/assistant_runtime/app/assistant/_prompt_builder.py, src/assistant_runtime/app/assistant/interface.py, tests/unit/app/assistant/test_prompt_builder.py, tests/unit/app/assistant/test_prompt_record.py
The prompt builder no longer accepts available tools. Callers and tests use the revised signature.
Session-scoped prompt snapshots and context preparation
src/assistant_runtime/app/assistant/_session_store.py, src/assistant_runtime/app/assistant/interface.py, tests/compatibility/test_prompt_record.py, tests/unit/app/assistant/test_interface.py, tests/unit/app/assistant/test_prompt_record.py
Memory-only snapshots are scoped to their session, while database-backed snapshots retain the bounded cache and database lookup. Prompt inputs load after configuration and model resolution.

Streaming snapshots and telemetry

Layer / File(s) Summary
Run snapshot capture and persistence
src/assistant_runtime/app/streaming/_snapshot.py, src/assistant_runtime/app/streaming/_turn.py, tests/compatibility/test_codex_fast.py, tests/compatibility/test_usage.py, tests/unit/app/streaming/test_interface.py
RunSnapshot and its helpers capture messages and usage, persist assistant rows, and mark unanswered tool calls. Steering turns carry forward usage when they extend an assistant row.
Turn runner and service integration
src/assistant_runtime/app/streaming/_runner.py, src/assistant_runtime/app/streaming/interface.py, tests/unit/app/streaming/test_client_error_detail.py, tests/unit/app/streaming/test_interface.py
TurnRunner delegates snapshot and telemetry work to dedicated modules. Setup failures and steering turns use the updated helpers.
Error, debug, and trace telemetry
src/assistant_runtime/app/streaming/_telemetry.py, src/assistant_runtime/app/streaming/_runner.py
Telemetry helpers classify errors, build debug and terminal events, format messages, and save traces on a best-effort basis.

Background task execution

Layer / File(s) Summary
Background-turn scheduling and execution
src/assistant_runtime/app/tasks/_runner.py
BackgroundTurnRunner manages reservations, ordering, concurrency, cancellation, shutdown, and terminal outcomes.

Merge Risk: ⚪ Minimal · up to ebe3b

Malformed Codex schema entries no longer hide supported methods, and shutdown handles the process-exit race. Normal startup rollback and shutdown protect cleanup from cancellation, leaving no actionable merge-blocking risk in the supplied context.

Security Architecture Review

Security architecture risk: 🔵 Low · up to ebe3b

The refactor changes shared-resource ownership and shutdown behavior. No new security issue was confirmed, but incomplete before-and-after and recovery coverage prevents a minimal-risk assessment.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The inspected failure domain is the shared Codex subprocess and its active voice threads: reader termination closes every subscribed queue on that link. Provider activity uses the configured CLI account, while application history access is mediated by session ownership. No broader tenant, datastore, or environment compromise was established.

Security Findings and Attack Paths

  • inferred — No introduced or worsened attack path was established for the routed RPC extraction. Caller-controlled instructions, history, and SDP reach the provider through typed bounds, authentication delegation, and session-authorization hooks. This is a bounded assessment, not complete verification of configured authentication policy, provider behavior, or base-to-head exposure.

Trust Boundaries and Controls

  • observed — VoiceOffer forbids extra fields and exposes no provider credential, CLI command, sandbox, or approval-policy selector. Session authorization is rechecked after admission before history access. Provider authentication verifies ChatGPT account type, not a separate provider identity for each application principal. SDP receives nonblank and size validation; semantic validation remains outside the inspected runtime controls.

Resilience and Maintainability Implications

  • observed — VoiceService serializes allocation against shutdown, and lifecycle cleanup protects the reachable shutdown chain from caller cancellation. These controls do not make the private RPC stop method independently cancellation-safe. Usage diagnostics also lack the allocation lock, so complete shutdown coverage for already-admitted diagnostic requests remains unresolved rather than a demonstrated PR regression.

Type the token fields in CodexCliAuth as SecretStr.

CodexCliAuth holds access_token, refresh_token, and id_token as plain str. A repr() of the model, or a traceback or log that includes it, prints the raw tokens. The service passes this object around (sync_from_codex_cli calls _use_codex_cli_auth(tokens)), so accidental exposure is plausible.

Use SecretStr for these fields. Call .get_secret_value() in _use_codex_cli_auth. CodexSession.access_token has the same exposure, but I did not trace its consumers, so check them before changing it.

This is a hardening change. The current code does not log the tokens.

Based on learnings: "type sensitive fields (passwords, API keys, tokens, secrets) as SecretStr (or SecretBytes) instead of plain str."

Tolerate a process exiting between the shutdown timeout and kill, then reap it. Skip malformed schema variants without hiding supported methods in valid entries. Cover both CodeRabbit findings with focused regressions.

[from:assistant-runtime] Addressed both actionable findings in ebe3b7d: forced shutdown tolerates a process exiting before kill() and still reaps it; schema parsing skips malformed variants while preserving valid method names. The existing schema regression and a focused shutdown regression cover these paths. Full checks pass: 2,966 tests, one database-dependent skip.

The optional SecretStr suggestion is not included. These model fields retain their existing types, and the review confirms no current token logging. Changing the credential model types would alter consumers without correcting a demonstrated exposure in this cleanup.
