- Tasks and pipeline updates store an actor: "human" or "mcp:<client name>". The Tasks page labels agent-recorded tasks and the evidence export gains pipeline_set_by. - Review reasons are a column (page, needs_user, outcome_unknown) instead of matching the "MCP:" note prefix. - Notes no longer carry the "MCP:" prefix. Existing unreleased databases from the MCP branch keep their old notes. - Docs: approval flow, shared-platform hosts, receipt trust note, link to the official MCP site, docs map entry.
