Adds a Codeium (Windsurf/Cascade) appendix to docs/primitives-matrix.md, mapping scheduling, skills/rules, state, and maker/checker primitives, per #228.

Documented surface: Windsurf IDE / Cascade agent. Codeium's brand today mostly covers enterprise completion analytics, not the agent loop primitives — this is clarified in the appendix intro.
New appendix follows the same structure as the existing Aider/Zed/Gemini CLI appendices: a primitive-mapping table, a minimal transfer recipe, a week-one Daily Triage workflow example, and a verifier-pass example. Placed directly after "Editor transfer recipes" since it expands on the Windsurf column already summarized there.
Included an explicit "Honest gaps" row per the issue's acceptance criteria: no durable scheduler, no native maker/checker split, no first-class state-file convention — Windsurf's loop primitives are manual/session-driven today.
