Prepare TrialMatchAI 0.9.1 after merging the paper-artifact audit, explicit TREC unjudged-policy evaluation, and default-on eligibility assessment fixes.

Changes:

• bump package and lockfile versions to 0.9.1
• publish 0.9.1 release notes and retain an empty Unreleased section
• update README and documentation installation examples to pin 0.9.1

Validation:

• 30 focused evaluator, reproduction, CLI, and package tests pass
• Ruff passes
• uv lock --check passes
• strict MkDocs build passes
• source distribution and wheel build as 0.9.1
• installed wheel reports trialmatchai 0.9.1 and exposes trec-evaluate
