cli: scaffold hygiene — 'sm' CLI name in templates, uv.lock gitignored, kebab-case Postgres DB name #443

Small issues found scaffolding a real app with simple_module_cli 0.0.35:

1. Wrong CLI name. templates/module/__PACKAGE__/module.py.tpl docstring says "generated by sm create-module" and templates/workspace/README.md.tpl says uv run sm create-module .... sm is the .NET CLI; the Python one is smpy.
2. uv.lock is gitignored in templates/workspace/.gitignore and templates/host/.gitignore. For an application (not a library) the lockfile should be committed. The Dockerfile copies uv.lock*, so container builds currently resolve fresh, unpinned transitive versions (e.g. a new sqlmodel minor), and CI and prod can diverge from dev.
3. DB name is kebab-case. --db postgres writes .../forest-observation-system (from to_kebab_case(name)). That's legal but needs quoting in every CREATE DATABASE / \connect / psql call, and differs from the snake_case convention of the other DBs in the shared dev-services stack. snake_case (forest_observation_system) avoids that.
