Milestone 9: reconcile F7-LAS architecture diagrams

Summary

• replace legacy and semantically ambiguous graphics with reviewed executive and layer-specific F7-LAS control-loop diagrams
• show approval returning to the PDP, permit-only flow through PEP, and Layer 6 as the post-enforcement execution boundary
• separate deny/block from clarify/refer-to-human terminal outcomes
• document implementation boundaries and enforce diagram hashes, dimensions, and semantics

Validation

• clean-user acceptance passed with Python 3.12.14 and OPA 1.20.2
• 98 tests passed
• evidence verification and deterministic replay passed
• golden dataset passed: 4 scenarios, 1.00 success rate
• all repository validators and git diff --check passed
• historical whitepaper remains unchanged
