feat: add artifact-first verifier references

Summary

• add a structured verification artifact reference and store compact verifier digests alongside verification artifacts
• reuse the same artifact reference in glm verify, repair overlays, loop handoff summaries, and loop history records
• keep loop execution semantics intact while making repair and handoff flows consume artifact summaries first
