Tracking issue: #1398

Part of the file-write-safety series (#1375) — B2: per-task change journal. Stacked on #1404 (B1, per-write checkpoints).

What

  • Append-only changes.jsonl under the task's checkpoint dir — one entry per successful file write: path, operation, checkpointId (the B1 checkpoint id), and diffStats (additions/deletions from the already-computed approval diff where available).
  • Torn-tail repair on load — a crash mid-append leaves a truncated final line; loading discards it, parses the rest, and never throws, so the journal survives a crash with no corrupt load.
  • Gated by perWriteCheckpoints — the B1 master switch: off = no journal entries, writes behave exactly as today.

Tests

  • changeJournal.spec.ts — append format (one JSON line per entry), clean-tail load, torn-tail repair (truncated final line discarded, rest parsed, no throw), absent/empty journal → [].
  • Hook-level: a successful write through the B1 hook appends exactly one journal line referencing the B1 checkpoint id; with the setting off no line is appended.
  • Regression: the B1 suites (write/edit/applyPatch hooks, Task, ClineProvider) stay green.
  • Local gates: eslint 0, tsc 0, 100% patch coverage.

Hardening (post-review)

  • Partial flush on mid-patch failure — when a later hunk is rejected (a .rooignore access denial), the loop breaks instead of returning, so writes completed by earlier hunks still receive the B1 checkpoint, the journal entry, and the change card; the failed patch also keeps the consecutive-mistake counter from resetting.
  • Failed-move journaling — if a move's destination file is written but the source deletion fails, the move is reported as a failed tool error while the destination write is still journaled and checkpointed, so that state stays restorable.
  • Regression tests: "checkpoints the written subset when a later hunk is access-denied" and the updated failed-move test (the checkpoint covers exactly the destination write).

Content source of record

kind: branch - feat/change-journal-b2, refreshed onto current main by merge 1d2373d80 (upstream/main 9c069e94ef); formatting-only follow-up 86f41828b.

Budget (own delta at this head)

Own delta vs the B1 head (b01a334e4): 11 files, +1026 / -70. The GitHub stacked view is larger until B1 merges; merge order: B1 (PR 1404) first.

How to test

pnpm --dir src exec vitest run --globals --no-file-parallelism core/checkpoints/__tests__/changeJournal.spec.ts core/checkpoints/__tests__/checkpointJournal.test.ts core/tools/__tests__/writeToFileTool.spec.ts core/tools/__tests__/editFileTool.spec.ts core/tools/__tests__/applyPatchTool.execute.spec.ts core/task/__tests__/Task.spec.ts core/webview/__tests__/ClineProvider.spec.ts
node scripts/find-missing-translations.js

Expected: all green; src/eslint-suppressions.json unchanged.

Verification at this head (25fe557a8)

  • CI: 19/19 checks green at head (incl. compile with the new format:check step).
  • Local: ESLint --max-warnings=0 clean on all 14 touched src files; tsc --noEmit clean; src vitest 500 passed / 5 skipped / 0 failed (7 files); webview-ui vitest 27 passed (2 files); post-format re-run of the four reformatted spec files: 88 passed / 5 skipped / 0 failed; translations check clean; model-check 9/10 locally (10th = known local Node 25 + tsx env defect only; green in CI).
  • Prettier: clean on every file in the unit diff. src/eslint-suppressions.json: unchanged.

Updated this round (refresh onto main 9c069e94ef)

  • 1d2373d80 - merge upstream/main. Same two test-file conflicts as B1; the ClineProvider.spec.ts pre-merge blob is byte-identical to B1's, so B1's resolution was reused verbatim (both sides' tests kept); ExtensionStateContext.spec.tsx takes main's DEFAULT_WRITE_DELAY_MS placeholder shape and keeps the perWriteCheckpoints key.
  • 86f41828b - formatting-only: upstream main added a pnpm format:check step to the compile job (new gate); five files in this diff predate the gate and were reformatted with prettier --write (no semantic change; tests re-run green). The same fix was ported to every sibling unit carrying these files.
  • ba5908ba2 - review follow-up (ported from PR 1404): task-start baseline is now skipped on history resumes (initiateTaskLoop gates on _isHistoryTask), matching the exactly-once-per-task design; regression test added in Task.spec.ts. Task.spec 177/177 green, tsc clean, Prettier clean.
  • 650382328 - review follow-up (ported from PR 1410): baseline skipped on already-aborted tasks (regression test added; Task.spec 178/178), and WriteToFileTool/EditFileTool now read perWriteCheckpoints through the shared DEFAULT_PER_WRITE_CHECKPOINTS constant. tsc clean, eslint clean, Prettier clean.
  • e443eb8bc - review follow-up (ported from PR 1410): the baseline block rechecks this.abort after the getState() lookup, so cancellation during the deferred state lookup skips the baseline save; regression test added (Task.spec 179/179, negative control verified). tsc clean, eslint clean, Prettier clean.
  • 4c0bc15bc - review follow-up (ported from PR 1404): EditFileTool now awaits checkpointSave before the diff-state reset and queued-work handoff (fire-and-forget could race the commit and lose it at shutdown); held-promise regression test added (negative control verified). Tool specs 102 passed / 5 skipped, tsc clean, eslint clean, Prettier clean.
  • 7b85cf223 - port: aborted tasks no longer dequeue queued messages (handoff sites guarded at the Task layer after the awaited-checkpoint change); regression test added. Task spec 180 passed.
  • 82639ccd5 - Stryker-directive sweep: removed the seven carried // Stryker disable next-line comments in Task.ts's streaming/telemetry region (comment-only). Equivalences: event.name always a string on those paths; streamingKey never undefined there; getStreamingKey fallback unreachable defensive code. eslint/tsc/Prettier clean.
  • a3fb016db - refresh onto upstream/main (58341d8 lint cleanup) and fix the unused-variable fallout the merge exposed: dropped the unused ChangeJournalEntry import in the checkpoint index and renamed an unused test stub arg (_uri). Full-src eslint 0, tsc 0, journal spec 8 passed, Prettier clean.

Review-gate re-trigger (2026-08-30): empty commit 7b83143 (no code change) re-runs CI and CodeRabbit current-head review under the org new PR review gate; the code head remains 93a8329.

Visual regression note (2026-08-30): The advisory extension-host-visual check (chat-dark sidebar snapshot) fails on this branch with a deterministic 973-pixel delta localized to the completion-result area. Root cause: the B1 task-start baseline checkpoint (per-write checkpoints default on, allowEmpty) adds a suppressed checkpoint_saved row to the message history, which getCompletionCheckpoint() picks up, so SeeNewChangesButtons render below the completion row; the suite baseline (recorded pre-FWS in #1426) has no such buttons. Every other pixel is identical, and the delta is pixel-identical across all branches containing B1 (1404/1406/1410/1411/1412/1413), while non-B1 branches are green. This is a new-expected-behavior vs stale-baseline mismatch rather than a rendering regression. Flagged for maintainers: either update the electron-chat-dark-sidebar.png baseline on the B1 branch or adjust the scene/component behavior. Advisory check only — it does not block the required gates.

  • loadChanges now validates the runtime shape of every parsed journal line and skips non-entry-shaped JSON (torn append leaving a bare number or null, manual edits, missing string path/checkpointId, unknown operation), per the review thread; regression test covers a torn-but-valid line plus four other malformed shapes (negative control: reverting the check reddens exactly that test). eslint/tsc/Prettier clean.
  • Refreshed onto upstream/main (tool-call state scoping): ToolCallbacks gained a required toolCallId; the execute spec's callback fixtures pass it (test-tool-call-id, matching main's own spec convention). No production-code change. Full-src eslint 0, tsc 0, execute/journal/Task specs 226 passed.
  • Doc/test-name-only commit: the journal entry contract comment now describes the written-subset journaling of partially failed patches, and the execute-spec describe block is renamed to match the subset-checkpoint tests it contains. eslint/tsc/Prettier clean, spec 26 passed.

Persistence Integrity walkthrough row - fixed

appendChange now (1) repairs a torn tail - an unterminated final line is terminated before the new entry is appended, so a crash-truncated journal no longer splices the next entry into the fragment and lose both - and (2) fsyncs each append, so a durable checkpoint implies a durable journal entry. Regression tests: append-after-torn-tail loads both the prior valid entry and the new entry; clean-tail appends insert no blank line. Negative control: disabling the tail repair reddens exactly the torn-tail test (1 failed / 23 passed), restored byte-exact.

The proposed whole-file atomic-rename/serialization redesign is not adopted: the journal is advisory metadata (rollback reads checkpoint commits; loadChanges tolerates malformed lines by design), per-append fsync removes the durability gap, and whole-file replacement would trade the single-syscall append for an O(n) read-modify-write on every write.

  • Review follow-up on the journal hardening: the tail-inspection catch now suppresses only ENOENT (fresh journal) and propagates other stat/open/read errors, and the append loops on FileHandle.write until the whole record is written before syncing (a short write can no longer persist a torn entry under a reported success). Two regression tests; negative controls each redden exactly their test, byte-exact restore. eslint/tsc/Prettier clean, checkpoints suites green.

Upstream refresh (this push)

upstream/main advanced (threshold commit for the series), leaving the branch conflicting with main on the regenerated e2e screenshot asset only; merged upstream/main into the unit branch and resolved apps/vscode-e2e/src/visual/__screenshots__/electron-chat-dark-sidebar.png to main's version. The PR diff remains unit-only; no source hunks were altered by the merge.

Main refresh (f707b6d) + production knip gate

Merged upstream main f707b6dfe (clean merge; gates and affected suites re-run green). Upstream enabled knip --production, which fails test-only production exports. Response on this head:

  • loadChanges carries an @internal tag (knip 5.60 has no export-level ignore config; the tag is its native mechanism, documented at the export site): it is wired by the rollback unit later in the split chain. appendChange stays - it is production code in this unit.

Updated this round (row-13 walkthrough fixes at 43b49b771)

  • Journal directory fsync on creation: when an append creates the journal file, appendChange now fsyncs the journal's parent directory before the append is reported as committed, so the new file's directory entry survives a crash. Platforms that cannot open the directory for sync degrade to a warning, mirroring the rename path's tolerance list.
  • loadChanges no longer swallows real read failures: an absent journal is still an empty history, but any other read failure (permissions, I/O) propagates instead of being reported as an empty rollback history.
  • Catch-path write-metadata flush: each completed write is recorded through an onWrote callback at the moment it lands on disk, and the catch path flushes the accumulated checkpoint/journal metadata (awaited) so a throw from post-write bookkeeping cannot lose the record of a completed write.
  • perWrite setting wording: the description now states that a multi-file patch produces a single checkpoint (all 18 locales; labels and keys unchanged).

Each fix carries a focused regression test; negative controls verified exactly-1-red per behavior.

Row-14 wave (round 49)

  • Task-start baseline hardening (byte-identical across all five units): settings-read rejection falls back to default-on (task startup continues); the baseline-done flag is set only after an explicit disable/abort or a committed save, so a no-commit save is retried on a later loop entry. Two regression tests; negative controls each exactly-1-red.
  • getCheckpointService never stores an uninitialized service (byte-identical guard across all five units): failed Git check / initShadowGit resolves to no service and clears the initializing flag. New getCheckpointService.spec.ts; negative control exactly-2-red.

Row-15 wave (round 50)

  • Move journaling completes the rollback story: a completed move now journals its source deletion ({operation: "delete"}) after the destination write, so a step rollback restores the removed source; a move whose source unlink failed keeps the single destination entry (the source is still on disk). Builder logic byte-identical across B2/B3a/B3b/B3c; the path-driven rollback needed no change. Regression tests for both outcomes; negative control exactly-1-red.
  • Catch-flush ordering hardened: writeMetadataFlushed is set only when the flush proceeds to checkpointSave and perWriteCheckpoints is cached for the catch flush - a rejecting settings read between write and flush no longer skips persisting writes already on disk. Regression test + exactly-1-red NC.
  • Catch-flush failures are reported: a rejected catch-path checkpoint flush is no longer swallowed; the reported error keeps the original message and appends the persistence failure (original preserved as cause). Regression test + exactly-1-red NC.
  • This push also retriggers CI after the known webview-ui vitest teardown flake on the previous head (2086/2086 tests had passed; zero assertion failures).

Row-16 disposition (evidence-grade, no code change)

The Persistence Integrity walkthrough row at this head restates two claims, both already dispositioned or superseded by landed code:

  • "successfulChanges is updated only after each handler returns; the outer catch reaches without calling checkpointSave" - stale against this head. Every handler records its write through the onWrote callback at the moment the write lands on disk (ApplyPatchTool.ts:377 add, :445 delete, :617 move destination, :629 update - each with a comment naming the throw it survives), and the outer catch performs an awaited flush of the recorded writes (landed at 43b49b7, hardened at 78275a3: the flush flag is set only when the success path proceeds to checkpointSave, the settings lookup is cached for the catch, and a rejected catch flush is reported alongside the bookkeeping error). The row's cited ApplyPatchTool.ts:244-247 is now the awaited success-path flush itself. Regression coverage: "journals the recorded write when a handler throws after its write", "flushes the recorded write from the catch path when the settings read rejects", "reports the catch-path flush failure alongside the bookkeeping error" (negative controls exactly-1-red each).
  • "checkpointSave commits first and appends the journal afterward; append errors are logged and swallowed" - the registered disposition stands: the journal is advisory metadata (rollback reads checkpoint commits; loadChanges tolerates malformed lines by design), per-append fsync removes the durability gap, torn-tail repair closes the crash-splice window, and a journal failure deliberately never propagates to the checkpoint error handler (checkpoints stay enabled; index.ts:269-271). The transactional CheckpointPartialPersistenceError hardening of this window is owned by the B3a+ units and landed there; adopting whole-file atomic replacement in B2 was evaluated and rejected (O(n) read-modify-write per append) with rationale above.

Row-17 confirmation (REPEAT - registration confirmed, no churn)

The 03:05Z Persistence Integrity row drops the earlier write-recording claim - it now states explicitly that "The ApplyPatch write-recording and awaited catch flush address the earlier bookkeeping-throw path" (the Row-16 disposition accepted). The remaining claim is the commit-then-append window, the registered advisory-journal disposition above (per-append fsync, torn-tail repair, journal failure logged without disabling checkpoints, transactional CheckpointPartialPersistenceError owned by B3a+; the pending-record transaction redesign is the same whole-file-redesign family evaluated and rejected above). Verdict: REPEAT.

Summary

Summary by CodeRabbit

  • New Features

    • Automatically save checkpoints after successful file writes, including available change details. A multi-file patch creates one checkpoint.
    • Save a baseline checkpoint when a task starts by default.
    • Enable or disable per-write checkpoints in settings; they’re enabled by default.
    • Keep a record of file changes associated with checkpoints to support recovery.
  • Localization

    • Added translated labels and descriptions for the per-write checkpoint setting.
  • Bug Fixes

    • Avoid recording changes for failed writes or unchanged files.
    • Preserve records for successful writes when other operations in a multi-file change fail.
    • Skip processing queued input after a task has been aborted.

The change adds a default-on perWriteCheckpoints setting, task-start baseline checkpoints, and a per-task JSONL change journal. File-writing tools pass operation metadata and available diff statistics to checkpoint saves. The settings UI exposes the option in multiple locales.

Full details: Title check

Explanation

The title names the feature, the per-task change journal (changes.jsonl), and the torn-tail repair behavior. The changeset adds an append-only changes.jsonl journal with torn-tail repair on load and append, so the title matches the main change. The title includes a tracking reference (B2, #1375) that does not describe the change, and the 76-character length is acceptable but slightly long for a short title. The title is clear and specific enough for a reviewer scanning history.

Full details: Description check

Explanation

The description follows the repository template partially. It links a tracking issue (#1398) but does not use the template's 'Closes: #<issue>' line, and it lists the series context instead. It has a clear What section, a Tests section with commands and expected results, and a Pre-Submission-style verification record. It covers the how only partly: it omits the Documentation Updates choice, the Visual Snapshots decision for the UI-adjacent i18n and settings changes, and the Get in Touch field. The description also contains a long run of update logs, commit hashes, and review-round notes that make it hard to read. A reviewer can still identify the change and the test approach, so the description is mostly complete.

Full details: Security Boundaries

Explanation

The prior finding remains unresolved. src/core/checkpoints/changeJournal.ts:207-228 reads pending-marker entries without validating them. A marker containing {"state":"ready","entries":[null]} throws when recovery reads entry.path. src/core/checkpoints/index.ts:272-287 then skips the per-write checkpoint. The invalid marker remains and can block later saves.

Resolution

Validate every pending-marker entry before using or appending it. Warn and remove unreadable markers, then continue safely. Add regression tests for null, primitive, missing-field, invalid-operation, and invalid-path entries, including the checkpoint-save path.

Full details: Persistence Integrity

Explanation

A committed checkpoint can lose its journal metadata. In src/core/checkpoints/index.ts:277-280,291-356, the pre-commit marker contains no write metadata. If both ready-marker promotion and incomplete-marker persistence fail, the outer catch returns without propagating the error. changeJournal.ts:196-232,386-389 then discards the awaiting marker on the next load. A crash after the commit but before promotion has the same result. The test at checkpointJournalPromotion.test.ts:164-175 confirms that the double-failure path leaves only the non-recoverable marker.

Resolution

Persist a durable pre-commit intent with enough information to identify the committed checkpoint and reconstruct its journal entries. Reconcile that intent after a crash or promotion failure, or roll back the checkpoint. Propagate unrecoverable persistence errors to the write path. Add tests for a crash after commit and for failure of both marker writes.

Full details: Regression Evidence

Explanation

The new checkpoint setting still lacks required regression evidence. SettingsView.spec.tsx now tests a true-to-false toggle and a default-valued payload, but does not test a stored false reaching the control or an unset value reaching the payload fallback. The fallback test starts from a context whose perWriteCheckpoints default is true. The new checkbox and description in CheckpointSettings.tsx also lack a Playwright component snapshot. webview-ui/AGENTS.md:48 requires a committed snapshot for visible UI changes. The prior failure is therefore only partly addressed.

Resolution

Add SettingsView coverage for a stored false and an unset perWriteCheckpoints, including the displayed value and saved payload. Add a Playwright component visual test for the checkpoint settings surface and commit its reference snapshot.

Full details: Lifecycle Resource Cleanup

Explanation

The prior lifecycle race remains. The new baseline in Task.ts:3856-3873 can wait for checkpoint initialization while the task is disposed. checkpointSave then saves without rechecking abort (index.ts:256-292), and its listener can post a message and call task.say after disposal (index.ts:183-206). A restarted task can run checkpoint work at the same time. Initialization also remains non-single-flight: the listener clears the initializing flag before the service is assigned (index.ts:122-134,178-181), so the new baseline lookup can start a second service.

Resolution

Use a single in-flight initialization promise and keep it active until assignment. Invalidate initialization on disposal, remove service listeners, and release the service. Recheck cancellation before assigning a service, starting checkpoint work, and emitting checkpoint effects. Add regression tests for disposal during initialization and concurrent baseline initialization.

Assert the journaled path for a successful move.

The test verifies writeFile content and one checkpointSave call. It does not verify the journal payload. The tool maps a move to change.movePath for the entry path. Add that assertion so a regression in the destination mapping fails here.

As per coding guidelines, add focused tests for persistence or normalization.

guard, then reuse it there and filter ready marker.entries with it before

before writing the awaiting-checkpoint marker or replacing it with new ready

…ep the commit recoverable

Persistence Integrity (pre-merge check): checkpointSave caught and ignored a failed pre-checkpoint pending-marker write, so the Git checkpoint could commit without the durable marker that is supposed to survive a lost append.

- The awaiting-checkpoint marker is now a hard prerequisite: when it cannot be written durably, the save is skipped (no commit) instead of running the checkpoint anyway.
- After a commit the ready promotion is retried; when it still fails, the entries are appended directly and the awaiting marker stays as the recovery record. If neither can be written, checkpoints are disabled rather than reporting success with missing journal state.

Test updated: a blocked checkpoint dir now asserts the save is skipped (saveCheckpoint not called) and that the failure is logged. checkpoint suite 61/61; eslint/tsc clean.

265-277: Unresolved earlier finding: writePendingJournal still overwrites an unreconciled ready marker.

The current code does not reconcile an existing marker before it writes awaiting-checkpoint at Line 267. The same marker file is overwritten again at Line 303. A ready marker can stay on disk after a failed reconcilePendingJournal at Line 320. The next write-bearing save replaces that marker, and its entries are lost.

192-193: LGTM!

…rnal entries cannot be promoted

Persistence Integrity (pre-merge check): the fallback appended entries directly after a failed ready-marker promotion and could still return the committed checkpoint as successful when that append failed.

- The direct-append fallback is replaced by a dedicated durable incomplete marker (changes.incomplete.json) that carries the complete entries; loadChanges folds it in on the next read (idempotent, marker removed once the entries are durable).
- When the ready promotion fails twice, the save writes that marker and throws, so a committed checkpoint whose journal state is missing is never reported as successful; the existing error handler logs it and disables checkpoints.
- Tests: a new promotion-failure spec forces only the ready write to fail, asserts the checkpoint is not reported successful, checkpoints are disabled and the incomplete marker carries the complete entries; a second test proves loadChanges folds the marker into the journal and removes it. checkpoint suite 63/63; eslint/tsc clean.

…te marker entries at runtime

Review threads on Zoo-Code-Org#1406:
- checkpointSave now reconciles a marker left by an earlier crash or failed append BEFORE writing the new awaiting marker, so its entries are not overwritten; a test seeds a ready marker with an earlier entry and asserts the next save keeps both entries.
- Marker entries read back from JSON are filtered through a runtime shape guard (isChangeJournalEntry) in both the pending and incomplete reconciliation paths, instead of a static cast.
- Tests: unreadable/unrecognized/empty-ready pending markers are discarded (marker removed, no invented entries); the marker is serialized with exact state literals and no extra fields; ENOENT assertions use toMatchObject({ code: "ENOENT" }).
- Promotion-path regression coverage: retry-success, incomplete-marker fallback (not reported as success), fallback-persistence failure (awaiting marker remains, checkpoints disabled), and reconciliation failure (result returned, failure logged).

checkpoint suite 71/71; eslint/tsc clean.

extension-host-visual on this head: read-only adjudication attempt, verdict insufficient evidence (ours-leaning, chain-correlated). Two discriminating facts, both measured just now:

  1. main does not run this lane at all. Paged GET /commits/e943530c848624bd3054c960be1e4fb0b658509a/check-runs?per_page=100&filter=all over 4 pages: 300 check runs, 0 failures, and no run whose name matches visual/extension-host. So the lane cannot be called environmental by pointing at a red base — the base never runs it.
  2. The lane goes green on our content elsewhere in the campaign. It is success on the abort-cascade head ba2e9dbdc, which touches the same ClineProvider and webview side. So this is not a permanently broken lane; the red on feat(checkpoints): per-write checkpoints, task-start baseline, and perWriteCheckpoints setting (B1, #1375) #1404/feat(checkpoints): per-task change journal with torn-tail repair (B2, #1375) #1406/feat(checkpoints): per-step change cards and changeCardDetail setting (B3a, #1375) #1411/feat(webview): change cards UI and rollback buttons (B3b, #1375) #1412/feat(checkpoints): per-file and per-step rollback service (B3c, #1375) #1410 correlates with this chain (all five heads carry it, oldest first at feat(checkpoints): per-write checkpoints, task-start baseline, and perWriteCheckpoints setting (B1, #1375) #1404, which is the chain root), which is what a content-triggered failure looks like.

Run shape on this head: started 15:45 -> completed 15:59 (~14 min), failed step Run Extension Host visual tests, 0 check-run annotations, and job logs are 404 for the token in use (no Actions:read), so the suite output is not reachable from the API.

What would settle it, maintainer-side: (a) Actions:read so the failing job log can be read, or (b) a one-off run of this workflow for #1404's head (fc11d6d64) against main. Until then I am deliberately not classifying it as advisory.

Unrelated to this lane, for whoever books reviews: live-row counts (GraphQL reviewThreads filtered on isResolved=false AND isOutdated=false) are 0 on all five chain heads as of now — #1404 fc11d6d64, #1406 2a67969fc, #1411 029c25ac3, #1412 dd8b4fd6a, #1410 5e9f60820 (heads verified with git ls-remote against the fork, not upstream). Zero live rows is not the same as a cleared review decision: #1404's newest three reviews contain a CHANGES_REQUESTED at 2026-10-22T10:27 followed only by COMMENTED, with nothing approving over it.

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.

Show when per-write checkpoints are unavailable.

When enableCheckpoints is false, this checkbox can still appear checked and accept changes. getCheckpointService returns no service in that state, so no per-write checkpoint is created. Disable the control and explain its dependency on automatic checkpoints.

---

Failed pre-merge checks:
- Regression Evidence:
Add focused `SettingsView` tests for unset, `true`, and `false`
`perWriteCheckpoints` values. Assert that each value reaches
`CheckpointSettings` and appears in the `updateSettings` payload. Add a
Playwright component visual test for the checkpoint settings surface and commit
its required reference snapshots.

- Security Boundaries:
Validate every pending-marker entry with `isChangeJournalEntry` before
constructing keys or calling `appendChange`. Treat invalid entries as an
unreadable marker, warn, remove the marker, and continue without those entries.
Add regression tests for null, primitive, missing-field, invalid-operation, and
invalid-path marker entries, and verify that `checkpointSave` still follows the
intended safe failure behavior.

- Persistence Integrity:
If ready-marker promotion and incomplete-marker persistence both fail, do not
leave a committed checkpoint represented only by `awaiting-checkpoint`. Roll
back or mark the checkpoint incomplete in durable storage that reconciliation
will retain. Propagate the persistence failure to the write path, and add a test
that forces both marker writes to fail and verifies that the committed write
remains recoverable.

- Lifecycle Resource Cleanup:
Implement single-flight checkpoint initialization. Store an in-flight
initialization promise before the first await and return it from every caller,
including the baseline path. Keep initialization active until the service is
assigned. On failure, remove registered listeners and release the service. On
task disposal, invalidate the in-flight initialization, remove checkpoint
listeners, and release the initialized service. Check `abort` or disposal state
before storing the service and before checkpoint work or checkpoint effects.

103-104: Pending and incomplete markers are still not crash-durable.

writePendingJournal and writeIncompleteJournal write a staging file and rename it. Neither syncs the staging file before the rename. Neither syncs the parent directory after the rename. A crash after the rename can leave an empty or truncated marker. reconcilePendingJournal then discards that marker as unreadable. The ready entries are lost. This is the case the marker exists to survive. Write through a handle, call handle.sync() before the rename, and sync the parent directory after it. Reuse one directory-sync helper with appendChange.

Based on learnings: "After creating or renaming a file that must survive a crash, also fsync the parent directory's file descriptor."

Also applies to: 140-141

Source: Learnings

218-231: Marker entries are still not validated in reconcilePendingJournal.

reconcileIncompleteJournal filters entries with isChangeJournalEntry. This function does not. A ready marker with a null element makes entry.path on Line 224 throw. The marker stays on disk. Every later loadChanges and every later checkpointSave for the task then fails. Filter marker.entries with the guard before building keys.

Proposed fix

-	if (marker.state === "ready" && Array.isArray(marker.entries) && marker.entries.length > 0) {
+	const markerEntries =
+		marker.state === "ready" && Array.isArray(marker.entries) ? marker.entries.filter(isChangeJournalEntry) : []
+	if (markerEntries.length > 0) {
const existing = await readJournalEntries(globalStorageDir, taskId)
 		const seen = new Set(
 			existing.map((entry) => `${entry.path}\u0000${entry.operation}\u0000${entry.checkpointId}`),
 		)
-		for (const entry of marker.entries) {
+		for (const entry of markerEntries) {

Based on learnings: "flag missing runtime validation: TypeScript types vanish at runtime."

Source: Learnings

289-311: Directory sync does not cover directories that fs.mkdir just created.

appendChange syncs only checkpoints/. If recursive mkdir created checkpoints/ or tasks/<id>/, the directory entry of each new directory lives in its parent. Syncing checkpoints/ does not persist those entries. Sync the parent of each newly created directory. Extend the creation test to assert those syncs and their order.

Source: Learnings

196-233: Stability & Availability

The reported duplicate-entry race is not established. The seen set and the sequential ordering inside checkpointSave cover the single-save path the code runs. The second entry point, loadChanges, has no production caller in this checkout, so it cannot overlap a save today. The only way to reach overlapping same-task reconciles would be two concurrent journal-bearing saves, and the inspected call sites all await their saves inside tool handlers. The comment's proposed per-task promise chain depends on that unestablished premise, so it is not a supported change request.

178-192: The reconcile-failure test does not assert the persisted recovery record.

The test checks the returned commit, enableCheckpoints, and the error log. It does not read pendingJournalPath(...) after checkpointSave. A regression that drops the ready marker would still pass. Assert state: "ready" and the entry with checkpointId: COMMIT.

Add a visual baseline for the new control.

This adds visible settings markup, but the gallery has no checkpoint settings or SettingsView story. Add a gallery story and a committed *.visual.tsx baseline for this surface. (raw.githubusercontent.com)

As per coding guidelines, if a PR changes something “a user would notice at a glance,” add a visual snapshot in the same PR.

Answered, and left open deliberately: this row needs a maintainer decision, and the honest thing I can do from here is measure the surface rather than commit an unverifiable baseline.

Mechanism. A CheckpointSettings.visual.tsx story would be the first visual test of anything wrapped in SearchableSetting — that component appears in 0 of the branch's visual files, so the row wrapper, the label, the description line and the checkbox would all be new baselines, plus a story id in webview-ui/playwright/gallery/stories.tsx (terminal-settings is the nearest template). That is a new visual surface, not a test case.

The count that separates behaviour from pixels. Behaviour is already covered: CheckpointSettings.spec.tsx:80 (description text and test id), :93 (control test id), :106 (falls back to DEFAULT_PER_WRITE_CHECKPOINTS when the state is unset), :114 (posts updateSettings with perWriteCheckpoints: false), :123 (checked reflects the stored value). Pixels are covered zero times for this surface: the branch carries 28 *.visual.tsx files and 12 toHaveScreenshot call sites, and none of them touches SearchableSetting or the checkpoint settings component.

Job state, stated as history rather than as a fact. In the run attached to the head you reviewed, extension-host-visual (job 126024088263, run 41666775346) failed on three electron-chat-dark-sidebar.png diffs through locator('.part.sidebar') while webview-visual (126024088881) and theme-fixtures (126024088915) were success — so a new webview story joins the leg that was green, not the leg that was red. The lead's own read of the last 24 runs of that workflow at job level finds 21 completed legs with zero test failures, so I am not calling that red run stale; I will re-read the job on this push's run and report if the same three sidebar failures come back, because then the answer changes.

Why I am not implementing it on this box. It cannot run Playwright visual at all. A committed .visual.tsx plus png baseline written here would be a baseline nobody verified — the failure mode where the test passes because the author's own pixels were committed.

Question for the maintainers: do you want a visual job for the checkpoint settings surface in this PR (new story id, new gallery entry, committed screenshots for the theme set), or as a follow-up filed together with the other SearchableSetting rows so the first one sets the pattern for the rest? If you want it in this PR, name the theme set and I will take it as its own unit. This row stays open until you say.

@easonLiangWorldedtech Thanks for separating behavior coverage from visual coverage. I agree that an unverified baseline should not be committed.

My recommendation is to add the visual test in this PR, subject to the maintainer’s decision. The repository’s visual-test guidance calls for a baseline for a visible UI change. I recommend the existing visualThemes set: dark, light, high contrast, and high contrast light.

If Docker cannot run locally, webview-ui/AGENTS.md documents a fallback: push the test without baselines, dispatch the pinned visual workflow, inspect its image artifacts, commit the container-generated baselines, and rerun the job. A maintainer can dispatch it if needed.

One correction: CheckpointSettings.spec.tsx checks the cached-state setter; it does not verify the SettingsView updateSettings payload. That coverage remains separate. I have not checked the current visual job or determined the cause of the earlier extension-host failure.

### When a UI change needs a snapshot

Visual regression is screen-based, not line-based — the goal is a small set of durable "pixel receipts" for the surfaces users see first, not blanket coverage. Prefer covering:

Follow-up on the visual-leg point in my answer, because the re-run you asked for landed on this head and I would rather correct the record than let it stand.

The extension-host-visual leg is red on this head too (job 128547911683 in run 42420577786, step "Run Extension Host visual tests": 3 failed / 10 passed, all three "expect(locator).toHaveScreenshot(expected) failed" on locator('.part.sidebar') with Snapshot: electron-chat-dark-sidebar.png, and that is the only snapshot name in the log). Measured wider than my own two runs: the failure is recurring across unrelated branches inside the same workflow's last 200 runs - 8 failing legs, 4 of them this exact sidebar shape, on feat/prompt-history-picker (twice), feat/session-title, and this branch twice (2a67969fc7 and e12e961c6). The other four failing legs are different shapes (two renovate/npm-lmstudio-sdk-zod-vulnerability with 7 failed, two with no test tally at all), so that job fails for more than one reason and none of them is a checkpoint-settings story.

So the ask is bigger than this thread and smaller than it looks: the re-run request now covers three jobs across three branches, and we are not implementing Playwright visual baselines from a box whose Playwright runs cannot execute the harness. What the part of the ask that this row is about would gain is exactly what you asked for - the test is blocked on a new webview story, and webview-visual, the leg such a story would actually join, is success on this same head (job 128547911897). Leaving the thread open for a maintainer call on whether that story is coming; the accessibility-naming and saved-false coverage you asked for is on the branch at SettingsView.spec.tsx:542.

@easonLiangWorldedtech Thanks for checking. I agree that an unverified baseline should not be committed.

The reported webview-visual pass does not cover the new control without a story. The recurring sidebar failures merit separate investigation; their cause is still unverified. I have not independently checked the CI results or the SettingsView.spec.tsx coverage.

This thread can remain open for the maintainer’s decision on the story and verified baseline. Separately, the saved pre-merge assessment reports blocking security and persistence findings. Those need verification against the current head before merge.

…lushing

The success-path checkpoint lookup runs after the per-file handlers, so a handler
that throws in post-write bookkeeping (trackFileContext, after its own write) jumps
to the catch with the cached setting still undefined. The catch read undefined as
default-on and persisted a checkpoint plus a journal entry for a user who had
disabled per-write checkpoints. It now reads the live setting whenever the success
-path lookup never ran, and a read that itself rejects leaves the default-on
behaviour, so a settings failure can still never strand a write that is on disk.

The catch-path case was written first: with getState resolving
{ perWriteCheckpoints: false } and trackFileContext rejecting after the write, it
failed against the shipped code because checkpointSave had been called once. The
neighbouring cases - no checkpoint when the setting is off on the success path,
journal the write when a handler throws after its write, flush from the catch when
the settings read rejects, report a catch-path flush failure alongside the
bookkeeping error - stay green, and core/tools/ stays green.

Answering the codeRabbit thread on src/core/tools/ApplyPatchTool.ts:268 (Zoo-Code-Org#1406).
…sertions

The checkpoint and terminal payload assertions used objectContaining, which cannot
fail when a key is missing from the updateSettings payload at all: dropping
perWriteCheckpoints from the payload leaves every existing case green. The
fallback-defaults case now names the key, and a new case loads a stored true,
un-checks the box, and saves, so the payload has to carry the false the user chose
rather than the display default.

Test-only: SettingsView.tsx is untouched. On the shipped payload line, removing the
key fails both assertions, and replacing the value with the default constant fails
only the new case. The LogicalOperator mutant the mutation CI drew at :418
(?? changed to &&) leaves the suite green, because the webview state default means
the left operand is never undefined at submit, so that mutant is equivalent here.

Resolve a mutation gap

Add or strengthen a focused test that fails under the mutation. If the mutant is equivalent, request maintainer approval for the narrowest mutator-specific exclusion and explain why it cannot change behavior:

// Stryker disable next-line ConditionalExpression: normalized input cannot reach the alternate branch
const result = condition ? value : fallback

Broad all exclusions and exclusions without a concrete reason are reported as advisories.

checkpoint behavior.

---

Outside diff comments:
Review comments at @src/core/tools/EditFileTool.ts:
- Around line 468-480: Move the awaited per-write checkpointSave call before
result reporting in the EditFileTool flow, after the edit is saved and marked
with task.didEditFile. Keep its journal metadata and error handling unchanged,
and ensure checkpointing completes before pushToolWriteResult, pushToolResult,
or diff reset.

Review comments at @src/core/tools/WriteToFileTool.ts:
- Around line 188-200: Move the per-write checkpointSave call in the
WriteToFileTool flow to run after task.didEditFile is set and before
pushToolWriteResult or pushToolResult can report the write. Preserve the awaited
checkpoint and its existing arguments so a saved write gets a checkpoint attempt
even if reporting is cancelled or rejects.

---

Failed pre-merge checks:
- Regression Evidence:
Add SettingsView coverage for a stored false and an unset `perWriteCheckpoints`,
including the displayed value and saved payload. Add a Playwright component
visual test for the checkpoint settings surface and commit its reference
snapshot.

- Security Boundaries:
Validate every pending-marker entry before using or appending it. Warn and
remove unreadable markers, then continue safely. Add regression tests for null,
primitive, missing-field, invalid-operation, and invalid-path entries, including
the checkpoint-save path.

- Persistence Integrity:
Persist a durable pre-commit intent with enough information to identify the
committed checkpoint and reconstruct its journal entries. Reconcile that intent
after a crash or promotion failure, or roll back the checkpoint. Propagate
unrecoverable persistence errors to the write path. Add tests for a crash after
commit and for failure of both marker writes.

- Lifecycle Resource Cleanup:
Use a single in-flight initialization promise and keep it active until
assignment. Invalidate initialization on disposal, remove service listeners, and
release the service. Recheck cancellation before assigning a service, starting
checkpoint work, and emitting checkpoint effects. Add regression tests for
disposal during initialization and concurrent baseline initialization.

Serialize journal transactions per task.

After a task is cancelled and resumed, its old and new checkpoint saves can overlap. The new save can replace the old save’s ready marker before reconciliation, so the old save returns success without recording its entry. Serialize saves and journal reconciliation for the same task.

Review comment at @src/core/checkpoints/index.ts around lines 276 - 280:
Serialize checkpoint saves and journal reconciliation per task, keyed by
task.taskId, so an older save cannot have its ready marker replaced before its
entry is recorded. Ensure the serialization covers the full save-and-reconcile
transaction, reusing an existing per-task mechanism if available.

Answered on #1406 with the mechanism, and leaving this thread open: the ask is a design change, and answering it is not landing it.

Your model of the interleaving is the one I would defend. There is no lock in this area — a search for mutex|AsyncLock|withLock|semaphore across src/core/checkpoints and src/services/checkpoints returns zero hits — and the path really is re-entrant: Task.ts:2429 runs void this.checkpointSave(false, true) fire-and-forget, Task.ts:3873 awaits checkpointSave(true, true) for the baseline, presentAssistantMessage.ts:1200 awaits task.checkpointSave(true), and the patch tool flushes from both its success path and its catch path. Two of those can be in flight for one task, the pending marker is one file per task, so the last writer wins and the folds interleave.

Two measurements before proposing anything. This is not something the series introduced: git blame -L 2425,2432 at the previous head e12e961c61 and at the merge base 3ee042d91e attributes every line of that block, the fire-and-forget included, to 19f306e6580ecc769ad5e2b56a97ca83447e6e53 (Naved Merchant, 2026-08-05), which git merge-base --is-ancestor confirms is an ancestor of main tip 73931fe8be — pre-existing upstream behaviour this series inherits, not a race this PR opened. And the unit boundary: a per-task serialisation point is a new concurrency primitive for a subsystem that has none, and it changes the cost of every checkpoint save, so it wants its own change with its own tests rather than arriving inside a durability commit.

Mechanism agreed; the primitive and its owner (per-task promise chain vs a real async lock, write-owner vs checkpointSave) proposed as its own unit. Leaving the thread unresolved for a maintainer to weigh in.

@easonLiangWorldedtech Thanks for tracing this. The overlapping checkpoint calls predate this PR, based on your history check. The shared pending marker is new, so journal-entry loss is the PR-specific consequence.

I understand the preference to handle serialization as a separate change. This thread can remain open for a maintainer’s decision.

Would you like me to open a GitHub issue for the serialization change and concurrent-save regression tests?

Yes, please open it — a tracked issue is what I wanted this thread to leave behind, and a title that names both halves is better than my phrasing was.

Your correction is right and I want it on the record rather than implicit: the overlapping calls are upstream (Task.ts:2429 void this.checkpointSave(false, true), blamed to 19f306e6580ecc769ad5e2b56a97ca83447e6e53, an ancestor of main tip 73931fe8be), but the shared pending marker is new in this series, so journal-entry loss under two in-flight saves is this PR's consequence, not the pre-existing race's. My comment said "pre-existing" about the re-entrancy, which is accurate; read as a claim about the failure mode it was too broad, and the loss mode is the half that matters.

For the issue body I would want three things: (a) the primitive and who owns it — a per-task promise chain at the checkpointSave entry, or a lock owned by the write-owner; (b) the per-save identity, because one marker file per task cannot tell two transactions apart, which is the same object the sibling row on the awaiting marker raises; and (c) concurrent-save regression tests that fail against today's code rather than passing either way.

Keeping the thread open as agreed. If no issue appears within a day I will file it myself with the blame evidence and both thread ids in its body so the trail survives.

## Summary

Serialize checkpoint save and journal recovery operations for the same task, and give each save a distinct transaction identity. Add deterministic regression tests for overlapping saves.

In `src/core/checkpoints/index.ts`, a write checkpoint reconciles and writes a pending marker before saving, then promotes and reconciles that marker after the commit. `src/core/checkpoints/changeJournal.ts` uses one pending marker per task. If two saves overlap, one save can replace another save's marker before reconciliation and cause a committed checkpoint's journal entry to be lost.

Overlapping checkpoint calls predate this PR. The author traced the fire-and-forget call in `src/core/task/Task.ts` to commit `19f306e6580ecc769ad5e2b56a97ca83447e6e53`, which is an ancestor of main tip `73931fe8be`. The shared pending marker and the resulting journal-entry loss are introduced by this series.

## Required work

- Choose and document the owner and scope of the serialization primitive. Consider a per-task promise chain at `checkpointSave` or a lock owned by the write owner. Cover the full save-and-reconcile transaction, including coordination with journal recovery, and account for old and resumed task instances sharing a task ID.
- Define a per-save transaction identity. A single marker keyed only by task cannot distinguish concurrent transactions. Coordinate the identity and marker design with recovery of an `awaiting-checkpoint` intent, including how recovery distinguishes a committed save from one that did not commit.
- Add deterministic concurrent-save regression tests with controlled interleavings. The tests must fail against the current implementation and verify that each committed save retains its own journal entries. Include overlap across old and resumed task instances and marker promotion/reconciliation; cover recovery interactions where applicable.

… marker entries

The two marker writers staged a file with fs.writeFile and renamed it without ever
syncing it, so a crash could leave the marker name in place over content that never
reached it. Both writers now take a write-mode handle on the staging file, write the
whole payload, sync once, close, rename, and then sync the containing directory best
effort - the order appendChange already uses for the journal itself. On this Windows
box that directory sync opens with "r" and lands in appendChange's own EPERM
tolerance list, so it degrades to a warning there; the content sync is what carries
the guarantee, and the directory part is no stronger than the journal path's.

reconcilePendingJournal read entry.path straight off marker.entries. A marker that
parses can still carry a null or a half-shaped element, and the TypeError escaped
loadChanges with the marker left in place: every later write-bearing checkpoint save
then failed on the same corruption. The fold now runs entries through the same
isChangeJournalEntry guard the incomplete fold at the sibling reconciler already uses.

writeIncompleteJournal renamed over whatever marker was already on disk, so a second
failed ready-promotion dropped the first batch of an already-committed checkpoint. It
now unions with the entries the marker carries under the same
path\0operation\0checkpointId key, preserving first-seen order: the earlier marker's
entries keep their positions, the new batch is appended after them, and a repeated key
is stored once.

ApplyPatchTool recorded a move's destination write only after the source unlink and
its error report, so a rejected task.say stranded a write that was already on disk and
left the catch-path flush with nothing to record. The record now happens right after
the destination write, before the unlink attempt and before bookkeeping. The index.ts
comment that called the pre-commit 'awaiting' marker the recovery record for the next
read is corrected: it carries no entries and reconcilePendingJournal clears it, so
that branch cannot fall back on it.

Tests first, each red against the pre-fix code and green after it, with the
implementation reverted to its HEAD blob and restored from a recorded blob sha:
reconcile with [valid, null, half-shaped] appends the valid entry and still clears the
marker (was TypeError: Cannot read properties of null (reading 'path')); staged
content synced before the rename and the directory after it, for both marker writers
(was "expected [] to have a length of 1 but got +0"); a second incomplete write keeps
the first batch (was "expected ['first'] to deeply equal ['first', 'second']"); a move
whose unlink and error report both reject still journals the destination (was
"expected \"vi.fn()\" to be called once, but got 0 times"). Two further cases
characterise behaviour this commit deliberately does not change: the next read discards
an awaiting marker left behind by a host exit between commit and promotion, and
checkpointSave resolves undefined when the pre-commit marker write fails so the tool
catch path reports only its own error.

Counts from this same git diff --cached --numstat run: +110/-21 changeJournal.ts, +7/-4 index.ts, +8/-5 ApplyPatchTool.ts, +157/-1 changeJournal.spec.ts, +29/-0 checkpointJournal.test.ts, +51/-0 checkpointJournalPromotion.test.ts, +33/-0 applyPatchTool.execute.spec.ts.
