Background
A pregnant girl is scheduled for an appointment when mapped by a community health worker and if shes misses the appointment then the system notifies the community health worker to do a follow up with the pregnant girl.

User story
As a village health worker, I should be able to view all the girls that have missed their appointments so that I can follow up to have a clear picture of why she had missed her appointment and for further antenatal care support.
The mobile application should enable the health worker to fil in a follow-up form showing the reason why the girl missed her appointment.
The mobile application should enable the health worker to indicate how the follow up was done.

Acceptance criteria

• The follow-up notes should show up in the midwife application.
• The mobile application should automatically notify the community health worker about a girl's missed appointment.

DoD

• The follow-up notes should show up in the midwife application.
• Data submitted needs to be validated.
• Data should be reflected on the dashboard.

• One can move to the next page without filling in any fields.

• The VHT follow up form should begin by asking if the girl missed ANC.

• There is a need for the application to be able to send reminder notifications to users to carry out a follow-up activity with the girls. These are sent two days to the ANC appointment and the next day for the girls that may have missed.
