Adds unit test to assert throttler.schedule is used and includes existing integration test for concurrency.\n\nTests: tests/cli/throttler-schedule-spy.test.ts (new) and tests/integration/github-throttler-concurrency.test.ts (existing).\n\nCI: vitest
