fix(openai-base): pass the streamed apply_patch diff to the app - #1948
A streamed apply_patch call reached execute with diff: "", so an agent loop sent empty patches until it hit its iteration limit. This PR starts a user-run call when its item is added, and ends it with the full input when the item is done. If the response completes while the call is still in_progress, the run ends with a RUN_ERROR instead of running the tool with incomplete input.
🎯 Changes
userToolChunks in packages/openai-base/src/adapters/responses-text.ts emits TOOL_CALL_START once, when it first sees the call. It emits TOOL_CALL_END with the input only when the item status is not in_progress.
At response.completed, if a user-run call is still in_progress, the adapter emits RUN_ERROR (code incomplete-tool-call). A stateless stream (ChatGPT plan route, Copilot) can skip output_item.done and send an empty output.
Two unit tests in packages/openai-base/tests/responses-text.test.ts: the real streamed diff sequence, and the empty-output case.
An E2E wire route and spec (openai-apply-patch-wire) send the streamed sequence through chat() and check the input that execute gets.
A changeset for @tanstack/openai-base.
No docs change: the fix restores the documented behavior.

🚀 Release Impact
This change affects published code, and I have generated a changeset.
This change is docs/CI/dev-only (no release).
Root cause
Issue. Every streamed apply_patch call on the OpenAI Responses API reached execute with diff: "". Cause.userToolChunks emitted TOOL_CALL_START and TOOL_CALL_END together on response.output_item.added. At that point the item is in_progress and its diff is empty. The later output_item.done was ignored, because the call was already ended. Fix.TOOL_CALL_END now waits until the item is no longer in_progress, so it carries the full diff from the done item. A call that is still open at response.completed fails the run with RUN_ERROR.
Possible alternatives
Build the diff from the delta events. Collect apply_patch_call_operation_diff.delta like function-call arguments. The done item already has the full diff, so this adds a buffer for no gain.
Testing
Commands run
An agent-written repro (chat() + execute on a streamed apply_patch_call), on clean main and on this branch:
main (9ac67f3): AssertionError: expected [ '' ] to deeply equal [ '*** hunk\n-a\n+b\n' ]
this branch: Tests 2 passed (2)
A probe with no output_item.done and an empty completed output. Before the follow-up commits: TOOL_CALL_START, then RUN_FINISHED tool_calls, with no TOOL_CALL_END. After: the run ends with RUN_ERROR (incomplete-tool-call), and the new unit test passes.
vitest run tests/responses-text.test.ts -t "apply_patch_call|in_progress at completion|user tool twice": 4 passed.
Low risk. For a user-run call, TOOL_CALL_END now comes at output_item.done, not at output_item.added. An item with no status, or any status other than in_progress, ends at once, as before. To undo, revert this PR.
Summary by CodeRabbit
Bug Fixes
Streamed patch operations now deliver the complete diff, including when it arrives incrementally.
User-run tool calls now finish with their complete input after processing is complete. If the response ends while a call is still in progress, the stream reports an incomplete-tool-call error instead of running the tool with partial input.
Tests
Added coverage for streamed patch diffs, tool-call completion and errors, and end-to-end delivery.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
The adapter now waits until a user-run call is no longer in progress before emitting its end event. If the response completes while a call remains in progress, the adapter emits an incomplete-tool-call error. Unit and end-to-end tests cover streamed apply_patch diffs.

Summary
User-run call lifecycle and unit coverage packages/openai-base/src/adapters/responses-text.ts, packages/openai-base/tests/responses-text.test.ts, .changeset/openai-streamed-apply-patch-diff.md
userToolChunks emits TOOL_CALL_START once and emits TOOL_CALL_END with the input when the item is no longer in progress. If response completion finds calls still in progress, the adapter emits RUN_ERROR with code incomplete-tool-call and returns without RUN_FINISHED. Tests cover completed diff delivery and the incomplete-call case. The changeset declares a patch release.
A test route streams an apply_patch_call with diff events, runs the tool executor, and returns recorded inputs. The route tree registers the endpoint, and a Playwright test checks the resulting file-update operation.
Completed streamed patches now reach execution with their full diff. Failed or truncated streams can still leave a tool call displayed as unfinished; this bounded issue warrants owner awareness and follow-up.

The Responses API adds an apply_patch_call with status "in_progress" and
an empty diff, and streams the diff in before the item is done. User-run
calls were started and ended at output_item.added, so the complete item
at output_item.done was skipped and execute always got diff "".
Start a user-run call when its item is added, and end it with its input
once the item is no longer in progress, as function calls already do.

A stateless stream can skip output_item.done and send an empty output.
The deferred TOOL_CALL_END then never fired, and the run finished with
finishReason tool_calls and an open TOOL_CALL_START. End such a call with
the last input seen, and log it.

Handle streamed apply_patch diff deltas in the stateless fallback.
The fallback ends an in-progress apply_patch_call with the input stored from response.output_item.added, which has an empty diff. No handler reads response.apply_patch_call_operation_diff.delta, so streamed diff text is dropped when output_item.done is missing. Accumulate the deltas into the tracked call's input before the fallback ends it.

Assert the last input of the fallback TOOL_CALL_END in the empty-output test.
The test named ends an in_progress user tool call when the completed output is empty collects only chunk types and their order. The fallback comment says the call ends "with the last input seen". That input is the in-progress item's operation. No assertion checks it, so a regression that drops or changes this input would still pass.
Give the in-progress item a complete diff in the fixture. The assertion then checks that the last input is preserved, without asserting that an empty patch is valid input.

Close deferred user-run tool calls on terminal error paths.
When an apply_patch_call or another user-run call is still in_progress and the stream ends with response.failed, response.incomplete, a top-level error, or without response.completed, the adapter emits TOOL_CALL_START and no matching TOOL_CALL_END. The deferred-end flush at responses-text.ts:2447-2459 runs only on the response.completed path. The terminal-error branches do not reference unendedUserCalls.
The saved end for these calls holds the input from the in_progress item. That input can be incomplete. The fix must not emit it as the final input for the call. Choose the terminal state for these calls with that constraint in mind.
Scope of this finding
The ChatClient path returns before finalizeStream() after a RUN_ERROR. The started tool-call part therefore remains in a non-complete state.

Add failure-path tests for an in-progress user tool call.
The new test covers only response.completed. The response.failed and response.incomplete events, and a stream that ends before response.completed, also end the run while a user-run apply_patch_call can still be in_progress. Add a case for each path. Each case should send an in_progressapply_patch_call and assert that no TOOL_CALL_END carries that call's partial operation input. This protects against a regression that executes an incomplete patch on these paths.
