chore(docs): remove phase plan and review docs - #75

Tickets now live only in the GitHub project. Drops docs/PLAN.md and docs/REVIEW*, removes phase/roadmap mentions from READMEs and stale PLAN.md references from AGENTS.md, CLAUDE.md and docs.
