feat(proportion): hand palm pad + distal digit refine (0064) - #68

Raise palm thickness/pad so pad depth clears the 0062 wrist soft bead, and replace equal finger segments with PP>MP>DP length taper plus mild distal radius scale. Keeps 0048 fr/splay/groove and mitten fences.
