docs: settle the itd-101–104 grills; capture iss-145

Closes the 2026-07-27 maintainer grill of the four draft intents. Six decisions written into the drafts as acceptance criteria, remaining open questions, and a Grill Settlements section each; DECISIONS carries the roll-up:

• itd-103 — guard fails open loudly (health in ahoy status); blocker/warn tiers with committed-only overrides, no in-session escape; shell-token-aware matching in command position only, with an itd-81-style calibration corpus (known-good ≥40%, TNR floor) gating registry admission; eval/shell-c payloads a documented v1 gap.
• itd-101 — refresh is manual with ahoy/release-preflight nagging (scheduled CI later, own sign-off); staleness warns at 180d in docs lint and blocks only the release gate at 365d; human verifications age on the same clock; the spc-15-deferred row-has-footnote rule is homed here.
• itd-102 — identity lives in a parseable markdown block (config records its location; markdown stays the single source of truth); the positioning check is warn-tier, per-repo upgradeable, and never rewrites a surface.
• itd-104 — ideate is an optional verb, never a pre-capture gate; the adversarial leg is fresh-context, off-policy, unknown-authorship.

Also lands iss-145 (receipts-cannot-gate-maintainability — candidate principle from the software-factories critique; adoption is the maintainer's call).

All four drafts are now plan-ready under the itd-1 discipline. Record-only; maintainer merges.
