Adds a sanitized entry for the wrong-machine hardening incident. The entry identifies capability-to-referent collapse, includes a steelman and A/U/T/D score, records the model as gpt-5.6-sol with high reasoning effort, and adds GPT-5.6 Sol to the frontier-tier mapping. Verified with the repository build script: 31 entries rendered successfully.
