docs(ssot): name every feature contract ssot_function_definition_<Topic> - #240

Unify description/definition and casing, give the I/O registry SSOT a real title, and rename the Stage-1 Tutor quality SSOT to TutorQualityGates so it is no longer confused with TutorQualityEvaluation. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
