Avoid exposing an unlocked empty preferences file
When the preferences file does not yet exist, .create(true) makes an empty file visible before the exclusive lock is acquired. A concurrent site-pref list can open that file, acquire its shared lock first, read an empty string, and fail JSON parsing instead of observing either the default or updated preferences. Use a sidecar lock that can be acquired before creating the data file, or make the locked reader treat an empty file as the pre-update default.

In `@src/local.rs`:
- Line 334: Update the cache write flow around the file open and exclusive lock
acquisition so opening does not truncate the file first. After the exclusive
lock is acquired, call set_len(0) and rewind() before serialization, preserving
the existing write behavior while preventing readers from observing a
prematurely truncated cache.
- Line 105: The cache lookup using open_locked_shared must not unlink an expired
entry while holding the shared lock, because cache_put may be waiting to acquire
an exclusive lock on the same file. Return Ok(None) without deleting the expired
pathname, or re-check the expiry under an exclusive lock before performing
deletion.

Keep expired cache files reachable while writers wait
When an expired entry is read concurrently with cache_put for the same key, this shared lock makes the writer wait after it has opened the same inode; the reader then removes the pathname on line 122, releases its lock, and the writer successfully writes the fresh value into the now-unlinked inode. On Unix the subsequent cache lookup therefore misses the response that was just cached. Avoid unlinking while holding only the shared lock—for example, acquire an exclusive lock and revalidate before deletion, or retain the expired file so the writer can truncate it in place.

all five open issues in one go. two are concurrency bugs in the local state files, three are validation and output contracts.
what was wrong
site-pref setcalls could silently drop each other's domains, and concurrent searches could interleavehistory.jsonllines into unparseable garbage. every local file access is now under an advisory lock, and site-pref mutations run read-modify-write under one exclusive lock ([bug] Concurrent writes to site-preferences.json can silently overwrite and lose domain preferences #176, [bug] Unlocked concurrent appends to history.jsonl can interleave and corrupt JSON lines #177)summarize --filterdied on the first bad item with zero output whileextract --filterstreams per-item envelopes. summarize now streams{"input", "ok", ...}lines and ends with the same aggregate failure + exit 1 (Inconsistent failure semantics between summarize --filter and extract --filter #178)fastgpt --web-search falsesailed through to an upstream that does not support it. rejected client-side with the reason (FastGPT accepts --web-search false despite being unsupported by upstream API #179)--generate-completionwith a subcommand exited 1 through manual checks. it goes through clap's conflict path now, so exit 2 like every other usage error (Top-level --generate-completion exits code 1 instead of clap usage error code 2 when used with a subcommand #180)proof
