[Feature] Vaccination record improvement #1005

Problem

Currently managing the Vaccination record is a bit rough:

  • Documents can only be tied to a single vaccine (so if I got a printed record of my childhood vaccination I cannot link it to all of the vaccines).
  • Documents have to be from the same date as the vaccination (same issue as above).
  • Adding custom vaccines (particularly for older ones) only ties them to 1 disease, so for example I cannot add "DTP" to the list and select which diseases it vaccinated against, I have to use DTaP with a note, or manually add a vaccine for each of the 3 diseases with the correct name. Ideally we could add in custom entries, or maybe give custom names to the existing list?
  • Similarly custom named vaccines don't let you set a validity period like the built in ones have.
  • In the regular view the information on whether a vaccine needs to be renewed is not surfaced at all, should be visible and highlighted on the last dose (or the heading of the section) where it's out of date.

Proposed solution
