fix: align DANFSe v2 output with production layout

Summary

Aligns the generated DANFSe v2 and DPS payload with the current national production layout observed in the official NFS-e output and NT 008.

Changes

• support optional provider phone/e-mail in the DPS
• render municipality/environment, emitter, status and purpose metadata
• use the official Contribuições Sociais - Retidas wording and retention description
• keep the IBS/CBS section visible in DANFSe v2 even when the authorized XML has no IBSCBS group, using - for unavailable fields
• expose IBS/CBS totals in the total-value block
• move approximate tax disclosure into Informações Complementares
• derive the displayed IBS/CBS exclusion/reduction amount only from already-authorized ISS/PIS/COFINS values; authorized IBS/CBS totals remain authoritative

Evidence

Compared against a real production NFS-e/DANFSe generated by the national emitter (DANFSe v2) and the current NT 008 requirements.
