Summary

Regenerates .github/workflows/README.md's "Set Up Environments" section
from the actual environment: bindings in .github/workflows/*.yml,
instead of a stale, invented list.

Root cause

The README instructed an operator to create aws-lambda-{dev,staging,prod},
aws-fargate-{dev,staging,prod}, gcp-{dev,staging,prod} and
azure-{dev,staging,prod}. Only the aws-fargate-* family matches an
actual environment: binding. deploy-aws-lambda.yml binds plain
dev/staging/prod; database-migration.yml binds
aws-db-<env>/gcp-db-<env>/azure-db-<env>, which the README omitted
entirely; and deploy-gcp.yml/deploy-azure.yml/rollback.yml bind names
this PR's siblings (#139, #140) are also correcting. An operator following
this README would configure required reviewers on environments nothing
binds to, while the credentialed jobs kept auto-creating theirs bare with no
protection -- this is audit finding A14-007, cited directly in #141's body.

Fix

Regenerate the "Create environments" list from the actual environment:
expressions, note which workflows share an environment rather than having
their own (cleanup-staging.yml reuses staging, destroy-fargate-dev.yml
reuses dev; three of rollback.yml's four jobs reuse the deploy
environments per #139, while rollback-aws-fargate pairs with
deploy-aws-fargate.yml's own aws-fargate-<env> instead), and add a note
that each cloud's OIDC trust policy must also allowlist the
environment:<name> subject for every name listed -- and does so three
different ways per cloud (Azure: a github_environments Terraform variable;
AWS: a literal sub list inlined in role.tf; GCP: ref-based, doesn't gate
by environment at all) -- or the GitHub Environment binding alone does not
let the job authenticate.

This documents the state once #139 and #140 also land --
deploy-gcp.yml/deploy-azure.yml/rollback.yml still bind
compound/missing names on main until those merge. The aws-lambda,
aws-fargate and database-migration entries are already accurate today.

Updated after independent review of #367's final form: the first
version of this PR said "all four rollback.yml jobs reuse the deploy
environments" and had a garbled sentence in the dev/staging/prod
bullet. #367 was corrected to bind rollback-aws-fargate to
aws-fargate-<env> (matching deploy-aws-fargate.yml), not plain
dev/staging/prod like the other three rollback jobs -- this PR's text
now reflects that, and the per-cloud trust-mechanism note above replaced a
version that incorrectly implied all three clouds use a single
"github_environments variable" mechanism.

Regression test

Not applicable: documentation-only change, no code path to regress.

Verification

$ markdownlint .github/workflows/README.md
(passed via pre-commit's markdownlint hook, --fix, no findings left after)

Scope note -- this does not close #141

#141's actual ask is creating the environments listed here with
protection rules (required reviewers, branch policies) in the live repo,
plus hardening GCP's OIDC attribute_condition so it is no more permissive
than AWS/Azure's sub-based trust. Both are repo-settings / cloud-IAM
changes outside what a PR to this repo's files can apply:

  • Creating GitHub Environments and setting protection rules is an admin API
action this repo's CLAUDE.md says not to script (Repo settings (branch rulesets, environment protection rules, secrets) are admin actions. Don't try to change them via the API.).
  • Hardening the GCP attribute_condition in
    terraform/environments/gcp/ci-cd-permissions/github_oidc.tf is a
bootstrap-only Terraform module applied manually by a privileged human
(same as the Azure/AWS siblings), and there is no way to verify a CEL
attribute_mapping/attribute_condition change against a real GCP
Workload Identity Pool from this sandbox. Getting that condition wrong
risks silently locking out every legitimate GCP deploy with no local way
to catch it before a human applies it -- not a change to make
    speculatively.

I've left a comment on #141 with the exact remediation steps and labelled it
status/blocked for those two pieces; this PR carries the code-side part
that's safe and verifiable to land now.

Do not require every cloud trust policy to allowlist every environment.

The environment:<name> subject does not identify the cloud. AWS, GCP, and Azure jobs all use the shared dev, staging, and prod subjects. A job with id-token: write can therefore satisfy another provider’s subject-based trust policy if it requests that provider’s audience.

Limit provider-specific subjects to the environments used by that provider. Shared environment names still cannot isolate clouds. Use provider-specific environment names or a provider-specific OIDC condition. Keep the GCP repository/ref condition; it does not use environment subjects.

Suggested README correction
- Each cloud's OIDC trust policy must also allowlist the `environment:<name>` subject for every
- name above (`terraform/environments/{aws,gcp,azure}/ci-cd-permissions/`) or the bound job's
- cloud login step fails with `AssumeRoleWithWebIdentity`/`AADSTS70021` regardless of what's
- configured here -- see the `github_environments` variable in each module.
+ Each provider's OIDC trust policy must allowlist only the environment subjects used by jobs
+ that assume that provider's role. The shared `dev`, `staging`, and `prod` subjects are not
+ cloud-specific. Use provider-specific environment names or a provider-specific OIDC condition
+ if the trust policy must isolate clouds. Keep the repository/ref condition in
+ `terraform/environments/gcp/ci-cd-permissions/github_oidc.tf`; the GCP policy does not use
+ environment subjects.

…indings

.github/workflows/README.md's "Set Up Environments" section listed
aws-lambda-{dev,staging,prod} and azure-{dev,staging,prod} /
gcp-{dev,staging,prod}, none of which match any current environment:
binding: deploy-aws-lambda.yml, deploy-gcp.yml and deploy-azure.yml
bind plain dev/staging/prod, rollback.yml's four jobs do too (#139),
and database-migration.yml binds aws-db-<env>/gcp-db-<env>/
azure-db-<env>, which the README omitted entirely. An operator
following this guidance would configure required reviewers on
environments nothing binds to while the credentialed jobs kept
auto-creating theirs bare with no protection -- audit finding A14-007
on #141.

Regenerate the list from the actual environment: expressions in
.github/workflows/*.yml, note that cleanup-staging.yml and
destroy-fargate-dev.yml reuse staging/dev rather than having their
own, and add a note that each cloud's OIDC trust policy must also
allowlist the environment:<name> subject for every name listed or the
binding alone does not let the job authenticate.

Documents the correct end state once #139 and #140 also land
(deploy-gcp.yml, deploy-azure.yml and rollback.yml still bind
compound/missing names until those merge); the aws-lambda,
aws-fargate and database-migration entries are already accurate on
main today.

Part of #141 (declaring the environments themselves with protection
rules is a repo-settings/bootstrap change outside what this PR does;
see the issue comment).

… review

Independent review caught two problems left over from #367's final
shape:

1. The dev/staging/prod bullet had a garbled mid-sentence insertion
("deploy-aws-fargate's sibling compute platforms use their own
names (below)") and, now that #367 rebinds rollback-aws-fargate to
aws-fargate-<env> instead of plain dev/staging/prod, the "all four
rollback.yml jobs" claim was also wrong -- only three of the four
do. Rewrote the bullet to name the three rollback jobs that share
this group and call out that rollback-aws-fargate pairs with
   deploy-aws-fargate.yml's own aws-fargate-<env> group instead.

2. The trust-allowlist note implied all three clouds gate environment
names the same way via a "github_environments variable". Only Azure
has one; AWS's trust list is a literal sub array inlined in
role.tf, not a variable; GCP doesn't gate by environment/sub at all
(ref-based only, per #141). Replaced the generic note with the
three distinct mechanisms and where each one lives.

Verified: markdownlint clean.

Independent review + local verification: MERGE at a063e91 (after #367). The workflows README's environment list now matches every environment: binding in the repo, and names the three trust mechanisms correctly (Azure github_environments, the AWS sub list in role.tf, GCP ref-based). markdownlint clean.
