Closes #442

Already on main (not redone here)

The two items in the issue body were fixed by the maintainer and merged with Refs #442. The issue stayed open for the items below.

What this PR does (the items #596/#598 left open on #442)

  1. The enrollment code is spent too. fix(auth): accept each TOTP code once #596 noted this follow-up: "MFAEnable stays stateless: the enrollment code can be replayed once at Login within its window." Now MFAEnable passes the matched counter to EnableMFA, which sets mfa_last_totp_counter in the same conditional UPDATE that turns MFA on (WHERE mfa_enabled = false AND mfa_pending_secret = $2 AND expires > NOW()). There is no separate write, so the counter can't be left behind without the enable, or the other way round.
    • The counter is replaced, not raised. The old value belongs to the previous secret, whose codes no longer verify. Keeping a higher value would refuse the new secret's current code when a user disables and re-enrolls within one 30s step.
  2. Per-account key on Login's TOTP/recovery-code leg (from @cristim's comment). There is a new login_mfa bucket of 10 per 15 min, keyed by email, in addition to the per-IP login bucket. It applies only when the request has an mfa_code, so the password-only first leg and non-MFA users are unaffected. The key is mail.ParseAddress(email).Address, lower-cased: the same parse Login does, so display-name and case variants share one budget. It fails closed (503) on a limiter error, like checkRateLimitStrict. The key is registered in assertRateLimitKeysKnown.
  3. requireMFASession checks the session before the per-IP mfa bucket (same comment). Requests without a valid session get a 401 before either bucket and don't spend the per-IP budget of users behind the same NAT.

Behavior matrix

Case Before After
Code used for MFAEnable, replayed at Login in the same window (password known) accepted invalid_mfa_code (and counts toward lockout, as any replay does)
N concurrent MFAEnable calls with one code one wins (CAS) one wins, and the code is spent
Disable, then re-enroll a new secret within the same 30s step accepted accepted (counter replaced)
Login with mfa_code, IPs rotated, same account per-IP limit only 429 after 10 per 15 min per account
Login without mfa_code per-IP only unchanged; per-email bucket not touched
login_mfa limiter error n/a 503, Login not called
MFA lifecycle request with invalid session spends the per-IP mfa budget, then 401 401, no bucket spent
Valid-session MFA lifecycle requests per-IP, then per-user unchanged

Decisions for review

  • Per-email bucket runs before the password check. The handler doesn't know whether the password is valid before calling Login, and moving the check into the auth service would mean plumbing the swappable (in-memory → DB) limiter into it. As a result, a caller without the password can spend a victim's login_mfa budget and block their MFA step for 15 min. This is not a new DoS: the same caller can already lock that account for AccountLockoutDuration with 5 wrong passwords from rotating IPs, and keep it locked. The 429 doesn't depend on whether the account exists. Owner option: count only after the password passes, via a service hook.
  • Router context for item 3: MFA routes are AuthUser, so the router's requireAuth (sec(auth): enforce per-permission Constraints on execute-any/own, approve-any/own, runPlanned (SEC-01 follow-up to #1210) #60) already rejected requests with no credential before the handler ran. In practice the reorder closes the API-key-principal case (authenticated, but no session) and fixes the handler-level ordering. Users behind one NAT who have valid sessions still share the per-IP 10/15min mfa budget. I kept that budget (I didn't remove an auth control). Owner decision whether to drop the per-IP bucket for session-authenticated requests.
  • No migration. Column mfa_last_totp_counter (000104) already exists; no IAmJSD PR collision to check.
  • The EnableMFA store-interface signature gains totpCounter int64. Postgres is the only backend; both mocks (internal/auth/test_helpers.go, internal/mocks/stores.go) and the health-test stub are updated.
  • The integration fixture enrollMFA now enrolls with the previous step's code, so the existing tests that then sign in with the current step still get an unspent step.

Verification

All tests ran with AWS_CONFIG_FILE=/dev/null AWS_SHARED_CREDENTIALS_FILE=/dev/null, on macOS with Docker (testcontainers, real Postgres 16), at the pushed SHA.

  • go build ./..., go vet ./..., go vet -tags integration ./...: clean
  • go test -count=1 ./internal/auth/... ./internal/api/... ./internal/config/... ./internal/database/... ./internal/server/... ./internal/mocks/...: all ok
  • go test -p 1 -tags integration -count=1 on the same packages: all ok. One run failed TestIntegration_LoginLockoutExpiry once, during container setup under heavy host load. That test doesn't touch MFA. It passed in isolation and on a rerun of the full ./internal/auth/ integration package (ok 40.4s).
  • golangci-lint v2.10.1 run --allow-parallel-runners (with and without --build-tags integration) on auth/api/server/mocks: 0 issues
  • gocyclo -over 10 on the changed non-test files: none
  • git secrets --scan (1.3.0, --register-aws, isolated HOME, temp copy of changed files + .gitallowed): clean. bash scripts/test-git-secrets-allowlist.sh: 42 PASS, 0 FAIL.

New tests:

  • internal/auth/service_totp_enroll_db_test.go (integration):
    • The enrollment code is refused at Login, and the stored counter equals the enrollment step.
    • Re-enrolling in the same step after a disable is accepted.
    • 8 goroutines racing MFAEnable with one code: exactly one succeeds, and the code is then refused at Login.
  • internal/auth/service_mfa_enroll_counter_test.go: EnableMFA receives the exact counter of the code.
  • internal/api/handler_auth_login_mfa_limit_test.go:
    • Login is throttled per account across rotating IPs and email variants; other accounts are unaffected.
    • No mfa_code → the per-email bucket is not touched.
    • Limiter error → 503 and Login is not called.
    • An invalid session doesn't spend the per-IP mfa budget.

Fail-before / mutation probes (each applied, test run, then reverted):

Mutation Result
Drop mfa_last_totp_counter = $4 from EnableMFA (pre-fix behavior) 3/3 enrollment integration subtests fail

Summary by CodeRabbit

  • Security
    • MFA login attempts now share a rate limit by email, including across IP addresses and email capitalization variants. Password-only logins are unaffected.
    • Requests without a valid MFA session no longer consume the per-IP MFA rate limit.
    • The TOTP code used to enable MFA can no longer be reused to authenticate a login.

… per account

The two items in the LeanerCloud#442 body landed in LeanerCloud#596 (TOTP counter) and LeanerCloud#598
(lockout on lifecycle failures); this closes what was left open on the
issue:

- MFAEnable passes the enrollment code's TOTP counter to EnableMFA, which
sets users.mfa_last_totp_counter in the same conditional UPDATE, so the
code typed to turn MFA on cannot be replayed at Login inside its window.
The counter is replaced, not raised, because it belonged to the previous
  secret.
- Login's TOTP / recovery-code leg gets a per-account "login_mfa" bucket
  (10 / 15 min, keyed by the parsed, lower-cased email, fail-closed), so
rotating source IPs no longer resets the code-guessing budget.
- requireMFASession validates the session before the per-IP "mfa" bucket,
so requests without a valid session do not spend that IP's budget.

📝 Walkthrough

MFA login requests now use an email-keyed rate limit, and MFA endpoints validate sessions before applying the per-IP limit. MFA enrollment now records the matched TOTP counter when enabling MFA. Tests cover rate-limit behavior and enrollment-code replay.

Changes: MFA login and endpoint rate limits

Layer / File(s) Summary
Configure and enforce MFA rate limits
internal/api/rate_limiter.go, internal/api/handler.go, internal/api/handler_auth.go, internal/api/middleware.go, internal/api/handler_auth_login_mfa_limit_test.go
The default limits include 10 MFA login attempts per 15 minutes, keyed by normalized email. The login handler checks this limit before authentication. MFA endpoints validate the session before applying the per-IP limit. Tests cover denied attempts, limiter errors, and invalid sessions.

TOTP enrollment counter

Layer / File(s) Summary
Match and pass the enrollment counter
internal/auth/interfaces.go, internal/auth/service_mfa.go, internal/auth/service_mfa_enroll_counter_test.go, internal/auth/service_mfa_test.go, internal/auth/test_helpers.go, internal/mocks/stores.go, internal/server/health_test.go
The enrollment validator returns the matched TOTP counter, and MFAEnable passes it to EnableMFA. Store interfaces and mocks accept the counter.
Persist and verify the enrollment counter
internal/auth/store_postgres_credentials.go, internal/auth/service_credentials_db_test.go, internal/auth/service_totp_enroll_db_test.go
The PostgreSQL store records the enrollment counter when enabling MFA. Tests cover enrollment-code reuse, reenrollment, and concurrent enable attempts.

Merge Risk: 🟡 Moderate · up to 94806

The new per-account limit on MFA login attempts is spent before the password is checked. Anyone who knows a user's email can use up that limit and block the user's valid MFA sign-in for up to 15 minutes, even across many IP addresses. Moving the check so it runs after password verification should be resolved or explicitly accepted before merge. The enrollment-code replay fix and the reordered session check do not add a comparable concern.

Security architecture risk: 🟡 Moderate · up to 94806

Enrollment replay protection is strengthened, but an unauthenticated caller can exhaust another account’s MFA login allowance. The new allowance can remain exhausted after an existing account lockout expires. The demonstrated impact is account-level login availability, not access to protected data.

Retained concerns

  • High · security · observed: The new account-email budget is consumed before password verification and before locked-account refusal. An unauthenticated caller can exhaust it while the victim is already locked, leaving correct MFA login attempts blocked after the original lockout expires. Passwordless account denial predates this PR, but independently chargeable admission state worsens the recovery boundary.
Security review details

Security Blast Radius

  • inferred — The demonstrated denial is independently targetable by account email and affects completion of MFA login. Multiple known accounts can be targeted separately, but this trace does not demonstrate data access, privilege gain or a service-wide outage. Other email budgets are independent, password-only requests skip the added gate, and session validation does not consult this login allowance.

Security Findings and Attack Paths

  • observed — The retained finding follows public login input through email parsing and lowercasing into shared admission state before password validation. With the default limits, ten admitted requests carrying a nonempty MFA code exhaust the target email’s window; source-IP rotation can distribute them across IP allowances. No password or MFA proof is required to charge the budget. The IP key comes from request context, so this trace does not establish header-based IP spoofing.
  • observed — Account-lockout denial already existed at the base: wrong passwords increment failures and eventually lock the user. The relevant difference is recovery ownership. Locked-account refusal prevents further failure updates, while the new email budget is still charged before that refusal. Successful-login bookkeeping clears user lockout fields, not the independently stored rate-limit window.

Trust Boundaries and Controls

  • observed — The session-ordering candidate does not demonstrate a newly reachable unauthenticated MFA operation or additional session lookup on the normal router path. MFA routes require authentication before dispatch, store the resolved principal in context, and then require a session specifically. API-key-only principals are rejected. Session validation is database-backed, but that authentication work already precedes the handler-level IP gate.

Resilience and Maintainability Implications

  • observed — The database limiter atomically increments the shared key and resets it after window expiry; denied requests do not extend an unexpired window. Cleanup removes sufficiently old rows. The in-memory alternative uses a mutex but is process-local and can evict live entries at its cap, so it does not provide the same distributed persistence guarantee.

Hardening Proposals

  • proposed — Separate pre-authentication abuse limits from the account-owned MFA budget. Verify the password, or a short-lived server-issued MFA challenge bound to the verified identity, before charging that budget. Retain independent fail-closed IP admission controls so removing unauthenticated ownership does not weaken guessing protection.

Explanation

Issue #442 requires single-use TOTP counters and account lockout for MFA failures. The reviewed head includes matchTOTP with counter tracking, ClaimTOTPCounter with a conditional mfa_last_totp_counter < $2 update, and use across Login and MFA lifecycle operations. MFAEnable also stores the enrollment counter in its conditional enable update. MFA code failures call recordFailedLogin, and locked accounts are refused. The new tests cover enrollment replay, concurrent enable, re-enrollment, and MFA rate-limit behavior. The linked coding requirements are met.

Full details: Out of Scope Changes check

Explanation

The changes remain within issue #442's MFA security scope. Enrollment-code consumption extends TOTP replay protection. The login_mfa account bucket limits MFA-code guessing across IPs. Session validation before the per-IP MFA bucket corrects MFA endpoint throttling order. The changed mocks, fixtures, and tests support these implementations. No unrelated change is demonstrated.

Independent review verdict: CLEAN at 9480654

Reviewer: fresh Claude agent (Sonnet, high effort), not the author, cold brief, acting as a security reviewer. The PR body was treated as a claim. Coordinator: claude-cc-reviewer.

Blocking findings: none.

Verified:

  • Enrollment code is spent atomically: EnableMFA (store_postgres_credentials.go) sets mfa_last_totp_counter = $4 in the same conditional, parameterized UPDATE that enables MFA. Login's ClaimTOTPCounter requires counter > last. Replacing the counter on re-enroll is correct, because the old counter belonged to the old secret.
  • Per-account throttle: the login_mfa bucket is keyed by the email after mail.ParseAddress and lowercasing, the same as Login. It applies only when mfa_code is present, fails closed (503) on a limiter error, and is registered in assertRateLimitKeysKnown. The bound is 10 attempts per 15 min against a 10^6 code space with ±1 step (about 3e-5 per window), on top of the per-account lockout, so brute force is infeasible.
  • No auth regression: password-check order, error text and constant-time compares in Login are unchanged. The requireMFASession reorder (session first, then the per-IP bucket) does not weaken anything.
  • Migration: none needed. mfa_last_totp_counter already exists on main (000104).

Non-blocking notes:

  1. The per-email bucket is spent before the password check (rate_limiter.go ~:95), so anyone who knows an email can burn that user's MFA budget. The body discloses this and argues it adds no new DoS beyond the existing 5-wrong-password lockout, which I agree with. Counting only after the password passes would be cleaner.
  2. Whether the throttle holds across Lambda instances depends on the limiter backend: InMemoryRateLimiter is per instance, while DBRateLimiter is shared. Which backend production uses was not verified; the durable fallback is tracked in #109.
  3. Enrolling with a skew-future code (counter c+1) refuses the current-step code at Login until the next step. This is conservative and harmless.

What was attacked and how (local macOS; Docker Postgres for integration):

  • go vet ./internal/... is clean. go test -race -count=1 ./internal/auth/... ./internal/api/... passes 4339.
  • go test -p 1 -tags integration -run 'MFA|Enroll|TOTPEnroll' ./internal/auth/ passes with 0 SKIP. TestIntegration_EnrollmentCodeIsSpent covers enrollment code can't log in, re-enroll after disable, and an 8-way concurrent-enable race.
  • Mutation probes (and fail-before):
    • Dropping the counter write in EnableMFA makes TestIntegration_EnrollmentCodeIsSpent fail.
    • Disabling the login_mfa check makes TestHandler_login_MFALegRateLimitedPerEmail fail.
    • Changing < to <= in ClaimTOTPCounter makes the enrollment test fail.
    • A lowercasing probe was inconclusive (an unused-import build error).

Gate:

  • Labels are mirrored from #442.
  • CI must be green at this SHA.
  • The draft needs to be marked ready, and CodeRabbit must review once it is.
