Update Project Standards to 5.11.0

Summary

• update the Catalog 5 control plane from Project Standards 5.4.0 to 5.11.0
• advance all seven enabled packages within their existing major tracks
• preserve consumer-owned workflows, handoff knowledge, extension formatting, and package options
• declare the repository-explainer pipeline as a coverage-disabled Python source root
• record the pre-existing production-state test failure as handoff bug 008

Consumer impact

The repository now reconciles cleanly against Project Standards 5.11.0. Managed workflows and package artifacts use the current Catalog 5 definitions, while existing @v5 workflow pins and consumer-owned content remain intact.

• Check and Agent Pseudocode Validation reproduce the default branch's existing media-CI failures: the runner lacks FFmpeg and its shallow checkout cannot resolve the capture manifest's pinned historical commit
• the current and baseline Agent Pseudocode runs both report 27 failed, 265 passed, 5 skipped, and 5 errors
