📝 Walkthrough

This change adds organization-wide issue and pull request standards, repository validation checks, shared skills and repository profiles, and a workflow that synchronizes configured standards across repositories.

Changes: Organization Repository Standards

Layer / File(s) Summary
Issue and pull request standards
.github/ISSUE_TEMPLATE/*, .github/pull_request_template.md, labels.json, README.md
Issue forms add required fields and metadata. The pull request template adds evidence and pre-merge checks. Labels and inherited organization defaults are updated.
Repository validation and workflow audits
.github/actionlint.yaml, .github/workflows/ci.yml, .github/workflows/hygiene.yml, lefthook.yml, scripts/validate-skills.sh
CI and pre-commit checks validate configuration, skills, shell files, and workflows. The reusable hygiene workflow adds optional zizmor auditing.
Shared skills and repository profiles
skills/*, templates/AGENTS.md, templates/configs/*, templates/lefthook/*, README.md
Shared Agent Skills and language configurations are added. Lefthook profiles define checks for Bun, Node.js, Python, and universal repositories. Repository guidance documents the standards.
Fleet synchronization
fleet-manifest.json, .github/workflows/fleet-sync.yml, scripts/sync-fleet.sh, scripts/sync-labels.sh, README.md
The manifest defines repository profiles and settings. The workflow runs the synchronization script, which applies configured standards and creates or updates pull requests. Label synchronization can update existing labels when requested.

Merge Risk: 🟡 Moderate · up to 889fa

Fleet synchronization is likely to stop working after its first run against each repository: re-pushing the sync branch is rejected, and that failure halts processing for every remaining repository. Missing push credentials or a missing label can stop the run in the same way. The public audit form also invites detailed security reproductions that should go through private reporting. Fix the sync push and error handling before relying on fleet-wide rollout.

Security architecture risk: 🟡 Moderate · up to 889fa

The new audit form invites detailed proof of security-boundary failures through public issues, despite the private-reporting policy. Fleet synchronization also centralizes cross-repository write authority and has a repeat-publication weakness that can delay standards updates. Main-branch execution, repository filtering, dry-run behavior, and pull-request publication provide meaningful containment, but actual token permissions and downstream protections remain unverified.

Retained concerns

  • High · security · observed: The new audit form includes security-boundary failures while requiring runnable proof, raw output, location, and remediation through the issue flow. A reporter following this form can expose vulnerability details before private remediation. The separate private-reporting policy and contact link reduce confusion but do not exclude security vulnerabilities within the form itself. This reporting-path conflict is introduced by the PR.
  • Medium · reliability · inferred: Repeated standards publication can fail when the configured synchronization branch already exists. Each run shallow-clones the default branch, creates a local sync branch, and pushes with force-with-lease without first obtaining that branch’s remote tracking state. The lease therefore normally expects the remote branch to be absent and rejects an existing branch before the PR can be refreshed. This can strand pending updates to shared validation and agent guidance. Workflow serialization prevents overlapping workflow runs but does not repair this repeated-run condition; actual target branch state was unavailable.
Security review details

Security Blast Radius

  • inferred — Compromise of the trusted synchronization source or its write-capable execution could affect labels and proposed standards across the configured fleet, subject to the token’s actual permissions. Target PRs provide a separate adoption boundary for file changes, but not for direct label updates. Public audit disclosure exposes details to issue readers wherever this form is used; automatic copying of the form to every manifest target is not established.

Security Findings and Attack Paths

  • inferred — A reporter able to open an issue can follow the audit form’s security-failure classification and publish reproduction commands and output before a fix. An issue reader then gains access to those details without advisory access. This is a disclosure-routing failure, not evidence that the form executes submitted commands or bypasses repository authentication.

Trust Boundaries and Controls

  • observed — Fleet synchronization runs on selected main-branch pushes or manual dispatch, not pull-request events. Dispatch values enter through environment variables and quoted argument arrays, and repository selection only filters manifest entries. Manual runs default to dry-run, and a missing organization token forces dry-run.
  • observed — The workflow declares read-only contents permission and disables checkout credential persistence, but supplies ORG_SYNC_TOKEN separately as GH_TOKEN when present. The declaration does not constrain that separate credential. Its repository scope, label and branch permissions, dispatch authorization, and main-branch approval controls are not established by the supplied source.

Resilience and Maintainability Implications

  • inferred — The publication path does not provide an atomic fleet transition: direct labels, branch publication, and PR creation have separate failure points. Recovery must distinguish already-mutated labels, a pushed branch without a PR, and targets never processed. Workflow serialization and lease protection limit races, but do not supply rollback or demonstrate successful repeated publication.

Hardening Proposals

  • proposed — Exclude vulnerability details from the public audit form and direct security-related findings to private reporting before requesting proof. Bind fleet credentials to intended repositories and operations, explicitly obtain synchronization-branch state before lease-protected updates, and record per-target publication outcomes so interrupted runs can be reconciled.

repository loop continues.
- Line 116: Update the push logic around the sync_branch operation so
force-with-lease uses the current remote branch SHA even when no remote-tracking
ref exists in the shallow clone. Retrieve the SHA for refs/heads/$sync_branch

preserving the existing branch rebuild and loop behavior.
- Line 57: Before the main while loop in the sync script, configure GitHub

Run actionlint in the workflow audit job.

When audit_workflows is true, this job runs only zizmor. Zizmor warns rather than fails on input parse errors by default. An invalid workflow can therefore pass the new audit job, despite the promised syntax check. Install and run actionlint, or make parse errors fatal as part of an equivalent syntax gate. (github.com)

Scan the staged content, not the working tree.

If a developer stages a file containing a machine-local path and then removes that path only from the working tree, this hook passes while the commit still contains the path. Use git grep --cached here and in the four profile templates. CI runs later and does not stop that commit. Git documents --cached as the option that searches the index. (git-scm.com)

Based on learnings, the repository invariant is “No machine-local paths in tracked files.”

I'm had issues with lefthook duplicating changes when running it on a repository that didn't have any commits. However now that I have commits I've just had an issue where I staged most of the changes but left some unstaged comments about changes that I still needed to apply and work I was progressing with. However after running lefthook (`lefthook run pre-commit`) it returned the following error and almost all the uncommitted changes were gone (untracked files were fine; 2 files with changes but none staged were fine; all files with some staged changes retained only the staged changes)

    `│  > git apply -v --whitespace=nowarn --recount --unidiff-zero /workspaces/foo/.git/info/lefthook-unstaged.patch
I'm not sure if I even attempted it. I think I've recovered most of changes using undoing, resulted in the changes reappearing.

Same class of failure here on Lefthook 2.1.9 while testing a Husky/lint-staged migration. Repro shape: partially staged tracked file + mutating pre-commit formatter. Lefthook hid the unstaged patch, the formatter rewrote the same lines, then `.git/info/lefthook-unstaged.patch` could not be applied back cleanly. Removing `stage_fixed` did not prevent the hide/restore path in my test. I could not find a simple “don’t hide unstaged changes” or “fail before mutating if partial staging is present” option. We wanted Lefthook to simplify hooks, but this pushed us toward avoiding Lefthook for `pre-commit` entirely. A no-hide or fail-fast mode would make this much safer.

The 2.1.7 release as improved things, with changes like the ones linked #1400, where earlier 2.1.x releases were causing issues for me constantly. I've not had an issue yet since that update, but haven't used 2.1.8. or 2.1.9 yet.</excerpt>

### Description

When a repository contains partially staged files, Lefthook's pre-commit guard runs `withHiddenUnstagedChanges` (`git stash create`, saving `.git/info/lefthook-unstaged.patch`).

If the working tree or index has any file registered with `git add -N` (intent-to-add), Git's internal tree-writing engine fundamentally refuses `git stash create`:

    `error: Entry '<file>' not uptodate. Cannot merge.
    Cannot save the current worktree state
    Failed to save unstaged changes: exit status 1
    Error: failed to run the hook: exit status 1

<excerpt>citeturn0search0 [wordlim: 200] Crawled: last week; # `stage_fixed`

# `stage_fixed`

Default: `false`

Note

Works only for the `pre-commit` hook.

When set to `true` lefthook will automatically call `git add` on files after running the command or script. For a command if `files` option was specified, the specified command will be used to retrieve files for `git add`. For scripts and commands without `files` option `{staged_files}` template will be used. All filters (`glob`, `exclude`) will be applied if specified.

If the `git add` call fails, the hook fails too. Otherwise the commit would silently go through with the unfixed content.

#### Example

    `# lefthook.yml

    pre-commit:
      commands:
        lint:
          run: npm run lint --fix {staged_files}
          stage_fixed: true

<location>https://lefthook.dev/configuration/run/</location>
<excerpt>citeturn0search2 [wordlim: 200] Crawled: 5 days ago; * `{lefthook_job_name}` - current job/command/script name ... # the name of the file that holds the proposed commit log message. ... run: yarn eslint "{staged_files}" # will run `yarn eslint "file1.js" "file2.js" "[strange name].js"`

# `run`

This is a mandatory option for a command, which specifies the actual command to be run using the `sh` shell.

You can use files templates that will be substituted with the appropriate files on execution:

  * `{files}` - custom `files` command result.
  * `{staged_files}` - staged files which you try to commit.
  * `{push_files}` - files that are committed but not pushed.
  * `{all_files}` - all files tracked by git.
  * `{cmd}` - shorthand for the command from `lefthook.yml`.
  * `{0}` - shorthand for the single space-joint string of git hook arguments.
  * `{1}` - shorthand for the 1-st git hook argument (and so on for `{2}`, `{3}`, etc.)
  * `{lefthook_job_name}` - current job/command/script name

Note

Command line length has a limit on every system. If your list of files is quite long, lefthook splits your files list to fit in the limit and runs few commands sequentially.

#### Example

Run `yarn lint` on `pre-commit` hook.

    `# lefthook.yml

    pre-commit:
      commands:
        lint:
          run: yarn lint
    `

#### `{files}` template

Run `go vet` only on files listed with `git ls-files -m` command with `.go` extension.

    `# lefthook.yml

    pre-commit:
      commands:
        govet:
          files: git ls-files -m
          glob: "*.go"
          run: go vet -- {files}
    `

#### `{staged_files}`

Run `yarn eslint` only on staged files with `.js`, `.ts`, `.jsx`, and `.tsx` extensions.

    `# lefthook.yml

    pre-commit:
      commands:
        eslint:
          glob: "*.{js,ts,jsx,tsx}"
          run: yarn eslint {staged_files}
    `

#### `{push_files}`

If you want to lint files only before pushing them.

    `# lefthook.yml

    pre-push:
      commands:
        eslint:
          glob: "*.{js,ts,jsx,tsx}"
          run: yarn eslint {push_files}
    `

#### `{all_files}`

Simply run `bundle exec rubocop` on all files with `.rb` extension excluding `application.rb` and `routes.rb` files.

Note

`--force-exclusion` will apply `Exclude` configuration setting of Rubocop

    `# lefthook.yml

    pre-commit:
      commands:
        rubocop:
          tags:
            - backend
            - style
          glob: "*.rb"
          exclude:
            - config/application.rb
            - config/routes.rb
          run: bundle exec rubocop --force-exclusion -- {all_files}

#### `{cmd}`

    `# lefthook.yml

    pre-commit:
      commands:
        lint:
          run: yarn lint
      scripts:
        "good_job.js":
          runner: node
    `

You can wrap it in docker runner locally:

    `# lefthook-local.yml

    pre-commit:
      commands:
        lint:
          run: docker run -it --rm <container_id_or_name> {cmd}
      scripts:
        "good_job.js":
          runner: docker run -it --rm <container_id_or_name> {cmd}
    `

#### Git arguments

Prevent commits from containing multiple sign-offs.

    `# lefthook.yml

    # Note: commit-msg hook takes a single parameter,
    # the name of the file that holds the proposed commit log message.
    # Source: https://git-scm.com/docs/githooks#_commit_msg
    commit-msg:
      commands:
        multiple-sign-off:
          run: 'test $(grep -c "^Signed-off-by: " {1}) -lt 2'
    `

#### Rubocop

If using `{all_files}` with RuboCop, it will ignore RuboCop’s `Exclude` configuration setting. To avoid this, pass `--force-exclusion`.

Lefthook is a Git hooks manager. It is

  * Fast
  * Powerful
  * Simple

## How does lefthook work?

You

  * Create `lefthook.yml` configuration file
  * Run `lefthook install`

Lefthook installs the configured hooks into `.git/hooks/`. Hook is a simple script that calls `lefthook run {hook-name}` when executed.

## How to install lefthook?

The most common way is to use the package manager of your project, e.g. gem or npm package.

You can also install lefthook via Homebrew, `winget`, `yum`, `apt`, `apk`, `scoop`

## Example configuration

Run linters on `pre-commit` hook.

    `# lefthook.yml

    pre-commit:
      parallel: true
      jobs:
        - run: yarn run stylelint --fix '{staged_files}'
          glob: "*.css"
          stage_fixed: true

        - run: yarn run eslint --fix '{staged_files}'
          glob:
            - "*.ts"
            - "*.js"
            - "*.tsx"

# `fail_on_changes`

The behaviour of lefthook when files (tracked by git) are modified can set by modifying the `fail_on_changes` configuration parameter. The possible values are:

  * `never`: never exit with a non-zero status if files were modified (default).
  * `always`: always exit with a non-zero status if files were modified.
  * `ci`: exit with a non-zero status only when the `CI` environment variable is set. This can be useful when combined with `stage_fixed` to ensure a frictionless devX locally, and a robust CI.
  * `non-ci`: exit with a non-zero status only when the `CI` environment variable is not set. This can be useful in setups where the CI pipeline commits changes automatically, such as autofix.ci.

Fail closed when partial-staging protection fails.

Lefthook normally protects partially staged files. However, each preHook failure only logs a warning and continues. If saving, stashing, or hiding unstaged changes fails, shfmt -w can rewrite the working-tree file, and stage_fixed can stage its unstaged hunks.

Make partial-staging setup failure abort the hook, or remove stage_fixed from mutating formatter jobs.

Update the formatter hook containing `stage_fixed` and `shfmt -i 2 -ci -w
{staged_files}` so partial-staging protection failures abort the hook before the
mutating formatter runs; alternatively, remove `stage_fixed` from this mutating
job. Preserve formatting of staged files without allowing unstaged hunks to be

## gh auth login

    `gh auth login [flags]
    `

Authenticate with a GitHub host.

The default hostname is `github.com`. This can be overridden using the `--hostname` flag.

The default authentication mode is a web-based browser flow. After completion, an authentication token will be stored securely in the system credential store. If a credential store is not found or there is an issue using it gh will fallback to writing the token to a plain text file. See `gh auth status` for its stored location.

Alternatively, use `--with-token` to pass in a personal access token (classic) on standard input. The minimum required scopes for the token are: `repo`, `read:org`, and `gist`. Take care when passing a fine-grained personal access token to `--with-token` as the inherent scoping to certain resources may cause confusing behaviour when interacting with other resources. Favour setting `GH_TOKEN` for fine-grained personal access token usage.

Alternatively, gh will use the authentication token found in environment variables. This method is most suitable for "headless" use of gh such as in automation. See `gh help environment` for more info.

To use gh in GitHub Actions, add `GH_TOKEN: ${{ github.token }}` to `env`.

The git protocol to use for git operations on this host can be set with `--git-protocol`, or during the interactive prompting. Although login is for a single account on a host, setting the git protocol will take effect for all users on the host.

Specifying `ssh` for the git protocol will detect existing SSH keys to upload, prompting to create and upload a new key if one is not found. This can be skipped with `--skip-ssh-key` flag.

For more information on OAuth scopes, see https://docs.github.com/en/developers/apps/building-oauth-apps/scopes-for-oauth-apps/.

### Options

`-c`, `--clipboard`
    Copy one-time OAuth device code to clipboard
`-p`, `--git-protocol <string>`
    The protocol to use for git operations on this host: {ssh|https}
`-h`, `--hostname <string>`
    The hostname of the GitHub instance to authenticate with
`--insecure-storage`
    Save authentication credentials in plain text instead of credential store
`-s`, `--scopes <strings>`
    Additional authentication scopes to request
`--skip-ssh-key`
    Skip generate/upload SSH key prompt
`-w`, `--web`
    Open a browser to authenticate
`--with-token`
    Read token from standard input

### Examples

    `# Start interactive setup
    $ gh auth login

    # Open a browser to authenticate and copy one-time OAuth code to clipboard
    $ gh auth login --web --clipboard

    # Authenticate against github.com by reading the token from a file
    $ gh auth login --with-token < mytoken.txt

    # Authenticate with specific host
    $ gh auth login --hostname enterprise.internal`</excerpt>

## gh auth status

    `gh auth status [flags]
    `

Display active account and authentication state on each known GitHub host.

For each host, the authentication state of each known account is tested and any issues are included in the output. Each host section will indicate the active account, which will be used when targeting that host.

If an account on any host (or only the one given via `--hostname`) has authentication issues, the command will exit with 1 and output to stderr. Note that when using the `--json` option, the command will always exit with zero regardless of any authentication issues, unless there is a fatal error.

To change the active account for a host, see `gh auth switch`.

Configure a Git credential helper before the loop.

The workflow uses GH_TOKEN, but the later plain git -C "$clone_dir" push needs a Git credential helper. The default gh auth setup-git can fail when the token exists only in the environment. Force configuration for github.com.

The token must also have Contents: write and Workflows: write. The script pushes branches and can modify files under .github/workflows.

Before the main while loop in the sync script, configure GitHub credentials by
invoking gh auth setup-git for github.com with force enabled, so the later git
push uses GH_TOKEN authentication. Ensure the workflow documents or validates
that the token has Contents: write and Workflows: write permissions.

The pin rewrite skips @main refs, but the comment says it handles them.

The comment on Line 91 says the rewrite matches @<40-hex-or-main>. The regex on Line 94 matches only @[0-9a-f]{40}. A target workflow that uses Heretek-AI/.github/.github/workflows/hygiene.yml@main is not pinned to source_sha, so it keeps a mutable ref.

--force-with-lease with no remote-tracking ref fails as "stale info" once the sync branch exists.

The clone uses --depth=1, which implies --single-branch. The clone therefore has no origin/chore/sync-org-standards ref. A bare --force-with-lease compares the remote branch with the local remote-tracking ref. If that ref is missing and the branch exists on the remote, git rejects the push with (stale info).

The first run creates the branch. Every later run against a repository with an open sync PR then fails on Line 116. set -e aborts the loop, and the remaining repositories are not processed. This breaks the idempotent update flow described in the header comment.

The script rebuilds the bot-owned branch from the default branch on each run. One fix is to fetch the remote branch first so the lease has a value to compare. Another fix is to use plain --force.

Update the push logic around the sync_branch operation so force-with-lease uses
the current remote branch SHA even when no remote-tracking ref exists in the
shallow clone. Retrieve the SHA for refs/heads/$sync_branch from origin, then
pass an explicit branch-specific lease to the push while preserving the existing
branch rebuild and loop behavior.

A failed label sync aborts the whole run at gh pr create --label "org-sync".

Line 50 treats a failed label sync as a warning and continues. If the label sync failed, the org-sync label may not exist in the target repository. gh pr create --label "org-sync" then fails. Under set -e, this ends the loop for all remaining repositories. The branch is already pushed, but no PR exists for it.

Create the PR without the label, and then add the label as a separate step that is allowed to fail.

Review comment at @scripts/sync-fleet.sh around lines 142 - 149:
Update the PR creation flow in the branch containing gh pr create so creation
does not depend on the org-sync label; remove the label option from gh pr
create, then attempt to add the label separately with gh pr edit --add-label
while allowing that command to fail and emit a warning so the repository loop
continues.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Reject frontmatter without a closing delimiter.

If SKILL.md starts with --- and contains valid-looking name and description lines but no second ---, awk reads to EOF and the validator can exit successfully. Check for the closing delimiter before validating fields. The Agent Skills format requires frontmatter followed by the Markdown body. (agentskills.io)

Review comment at @scripts/validate-skills.sh around lines 29 - 36:
Update the frontmatter extraction logic in scripts/validate-skills.sh to require
a second closing "---" delimiter after the opening delimiter. Detect a missing
closing delimiter before validating name or description fields, emit the
existing validation error and set status=1, and continue rejecting that skill
instead of treating EOF as valid frontmatter.

Replace ./.github/workflows refs with \$/ syntax, forward OCR_LLM_TOKEN explicitly instead of secrets:inherit, and switch ocr trigger from pull_request_target to pull_request to clear hygiene Workflow syntax and security failure.
