Reassert always-on-top window state

What changed

• Reassert the native Windows topmost state once per Codex lifecycle poll.
• Preserve the user's always-on-top context-menu preference.
• Bump the application version to 1.1.1.

Why

After a Codex Desktop update, Windows still reported the widget as visible but the native topmost flag had been cleared, leaving it behind the Codex window.

Validation

• Clean Release build completed.
• Privacy scan found no local username, absolute build path, PDB path, or token pattern in the executable.
• Forced the widget to HWND_NOTOPMOST and verified it automatically returned to HWND_TOPMOST after the next poll.
• Confirmed the local Codex app-server child remained active.
