feat: open field descriptions on hover - #121

The help icon beside a form field's label only opened its description on click, which users did not discover. It now also opens on mouse hover and stays open while the pointer moves onto it; click and touch still work. The description no longer takes focus when it opens, so hovering the icon cannot pull focus out of a field being typed in. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
