Physical Intervention form enhancement for ear tagging #1602

Current Implementation and Limitations
The form does not have a section that asks what ear tagging was performed for that specific animal.

add a question:
- what ear tag was performed? (ex of possible answers : L, R, LL, LLR...) - should probably not be drop down as it can be different depending on the type of system

In results of that form, the question tattoo location was a row (but I was not asked that questions - and that question does not belong there, as this was the ear tag form not the tattooing form)
