Current incident — 2026-09-24 KST fresh owner read

ConceptWeave does not own central Actions admission, CodeQL verdict publication/GHAS permission, or organization ruleset mutation. Keep those repairs in ContextualWisdomLab/.github; keep semantic source heads stable unless a real leaf finding requires change.

ConceptWeave authority

#35 retains the trusted ${{ github.workflow_sha }} Product control plane, candidate-workload separation, distinct Product acceptance / Product metadata-only identities and non-cancellable evidence. SAST 35825043007 and Security 35825042856 are terminal SUCCESS. Required CodeQL 35825042996 remains a central lifecycle/GHAS identity failure rather than a ConceptWeave source finding.

Downstream .github run 35870670165 validated the exact #35/head/base/required-run tuple. Python 107322443235 and Actions 107322443261 passed exact-head materialization, CodeQL analysis and Medium+ SARIF gating, then failed at GHAS base/head configuration identity; settlement 107396154783 later failed at Settle exact CodeQL required run. GHAS credential/permission remains .github#2275/#2276; lifecycle/publication/recovery .github#1929/#2040; queue continuation .github#2356. Do not move #35 to manufacture another generation.

#46 retains the PostgreSQL 18 semantic repairs and reviews 5293650822 / 5295345693 for the schema-wide pg_class namespace gap. Current exact 4244f580... still lacks the retained native Product/Rust/PostgreSQL execution required to prove the existing RED before the minimal aggregate namespace guard is added.

Protected Product-governance owner

.github#2348/#2350 owns Product enforcement. Accepted control remains one dedicated organization branch/workflows ruleset scoped only to ConceptWeave repository id 1353201939 and protected/default main, bound to source repository id 1353201939, .github/workflows/product.yml, refs/heads/main; no repository-status fallback and no Product addition to global ruleset 18156473.

The post-Product-blob protected-ref finding 5294002721 now has a complete attributable RED -> minimum repair -> exact-head GREEN chain. Hosted RED 1a6ddce2d02df50dddeb81450ff05cb9778c94a9 / run 35944402217 proved evaluate POST / active PUT must remain zero when protected .github/main or ConceptWeave main advances during the final reviewed Product blob read. Production repair 97acdfeef0830d56c7b9b78226fec53de791ee8f added mutation-immediate protected-ref revalidation. Its first hosted run exposed only stale retained success-path call counts. Test-only successor 04414c5e30ba6aed2bf3d82268b356c6d6b1ad84 corrected those two expectations and is terminal owner GREEN in run 35983462019; validate 107580552666 completed SUCCESS on hosted runner 1002122732, while mutation/live jobs were skipped. No live Product-specific ruleset state changed.

Current .github#2350 exact is ordinary-forward test-only 1e8da05d5b0851da212b3e340e7085fb1de550ce, one commit ahead of that GREEN head. It is the separate 5288830215 + 5291871021 supported-canary RED candidate: retained tests now require an OPEN Draft Foundation canary to be valid and require the supported substantive pull_request:synchronize path not to depend on _latest_base_retarget() / base_ref_changed. Current production still rejects draft=true and still calls _latest_base_retarget(). Exact owner run 36010121000 is queued; preserve this exact until terminal hosted evidence. No blind/manual rerun, no-op wake, Ready/Draft manipulation, or preemptive production repair.

Research/semantic stack authority

Research Intake #9 is exact a67d9d66b35024d6f2155f50ee5c9fb7d2e1dbe9, OPEN / Draft / mergeable, but remains diverged from current Foundation #1 84e56256a794b11568c271818c55c899ecfcbb60; that stale-parent boundary remains the research root repair. Golden-set #10 is current bf6ad9a1c613bd4a36a237d914030997d5721b02, OPEN / Draft / mergeable on #9. #11 remains stale on historical #10 ancestry and must not be restacked before #10 acceptance.

#43 remains stale/conflicted against Foundation and #44 remains its exact child. Do not bypass Foundation/Product by copying protected-owner repairs into semantic branches.

docs/product-technical-gap-baseline.md on Foundation still contains stale historical Product/.github coordinates; currentize it together with Foundation's substantive post-Product reconciliation rather than moving Foundation for docs-only evidence reset now.

Required order

  1. Keep CodeQL lifecycle/publication/recovery in .github#1929/#2040, GHAS credential/permission in .github#2275/#2276, queue continuation in .github#2356, generic governance in #772/#1351/#1644.
  2. Preserve .github#2350@1e8da05... until run 36010121000 is terminal. If it gives attributable supported-canary RED, apply only the minimum production repair on an ordinary-forward head and obtain exact-head GREEN.
  3. Land #2350 source normally with no live Product mutation.
  4. Obtain terminal current-head central evidence for chore(ci): bootstrap Product pull-request workflow #35 and normally land the one-time Product producer bootstrap.
  5. Re-read protected Product, adopt only its immutable Contents blob SHA through ordinary reviewed .github source, create the dedicated ruleset in evaluate, and adopt returned identity.
  6. Ordinary/non-force reconcile Foundation feat: establish ConceptWeave foundation #1. Its substantive Draft pull_request:synchronize event is the evaluate-mode Product canary; promote evaluate -> active only after exact first-attempt Product success, exact workflow-rule PASS, reviewed blob, current protected refs and no later source movement.
  7. Reconcile Research Intake feat: classify Zotero research snapshot #9 onto the complete accepted Foundation, then propagate exact accepted parent state through feat(research): add steward golden-set evaluation #10/feat(research): add Zotero classification audit evidence #11 and the later research/write stack. Separately propagate Foundation through feat(client): add offline semantic release admission contract #5/feat(observation): define immutable PostgreSQL schema snapshot contract #6/feat(observation): add domain-separated v3 successor schema representation #45/feat(observation): add v3 relation-scoped index evidence #46 and prove the retained namespace RED before its minimal repair.

No force push, destructive rebase, self-approval, fabricated reviewer/status, review dismissal, source-neutral wake/no-op commit, blind/manual rerun, gate weakening, mutable foreign dependency, predecessor-evidence transfer, premature semantic publication, or live Product ruleset mutation is authorized.

Fresh admission evidence — 2026-09-01 17:44 KST run

The blocker is broader than the repository Product workflow. Exact PR head 1c982f246f0b13c814d463ae2911e1049f9c253d currently has 18 check runs. Repository Product run 33485782056 / rust-quality job 99785397728 is still queued with no steps. On the same exact SHA, repository Security Scan 33485781993 and SAST Semgrep 33485781967 are also still queued. Check-run inspection shows substantive Security jobs such as dependency-review, scorecard, and osv-scan queued with no execution; only control/helper jobs such as cancel-closed-pr-runs have completed as skipped.

The repository is covered by active organization ruleset 18156473 (CWL Central required workflows) on ~DEFAULT_BRANCH, requiring one approval, thread resolution, and centrally owned close-empty/OpenCode/scheduler/security/Strix/SAST/Noema workflows. No rule weakening or bypass is justified.

This pattern makes a single malformed Product YAML materially less likely: multiple independently defined repository/central lanes on the same new repository are waiting for runner admission. Continue investigating organization Actions capacity/concurrency, new-repository Actions policy/bootstrap, billing/spending, or runner-group admission. Until a runner executes the exact RED SHA, do not commit the publication-invariant production repair and do not transfer any predecessor check evidence.

    Fresh exact-head queue evidence — 2026-09-01

Revalidated the live foundation lane without using predecessor evidence.

    • Protected/default base remains main@f4f440dd58c77d7cd90dff8a1eb2eeb9a9940425.
    • PR feat: establish ConceptWeave foundation #1 is open, Ready, mechanically mergeable, exact head ec9b8125f86ab9b7545ba501f7b7c7c7d31c6783.
    • Product run 33491678897, job 99804362735 (rust-quality) is still queued; the job has no execution steps, so exact-revision verification, fmt, Clippy, tests, rustdoc, coverage, schema validation, lock freshness, and clean-tree validation have not executed.
    • SAST run 33491679017, job 99804366548 (Semgrep (multi-language SAST)) is queued with no execution evidence.
    • Security run 33491678932 remains queued; current exact-head jobs 99804366871 (osv-scan), 99804367009 (scorecard), 99804367217 (dependency-review), and 99804367227 (trivy-fs) are queued.
    • Central current-head lanes are also non-passing: required-workflow-bootstrap job 99804362864, close-empty 99804360382, noema-review 99804361237, and scan-pr-queue 99804413731 are queued; strix job 99804358861 is pending.
    • Organization ruleset 18156473 remains active on the default branch and still requires one approval, thread resolution, the seven central workflows, deletion protection, and non-fast-forward protection. No bypass or gate weakening is acceptable.
    • GitHub's public status currently reports Actions operational with no September 1 incident, so a public platform outage is not being asserted as the cause.

The same organization-level class is already owned by ContextualWisdomLab/.github#712. This issue is the ConceptWeave-local blocker/acceptance ledger while .github#712 owns capacity/billing/policy/concurrency RCA. Do not churn PR #1 or create no-op commits merely to retrigger the sole exact-current-head evidence; doing so would discard the current diagnostic coordinate and increase queue pressure.

The earlier SemanticCandidate publication invariant is repaired on the current PR #1 source and is not the current blocker. GREEN here remains infrastructure-specific: a fresh/current exact ConceptWeave head actually acquires a runner and executes exact-revision verification before tests/security gates, followed by terminal current-head results and normal review. Until then, queued/pending evidence remains incomplete.

Current-head admission evidence — supersedes earlier run IDs

Fresh leaf state now binds this incident to PR #1 exact head 7cc16b7eef6a6ac6465f39e8795388758031907d over main@f4f440dd58c77d7cd90dff8a1eb2eeb9a9940425.

    • Product 33502572630 → rust-quality 99839064519: queued, no steps materialized. The current leaf workflow is a single ubuntu-latest job with no matrix, custom label, environment gate, reusable-workflow call, or concurrency group, so the first non-progress boundary is before exact-SHA checkout/source execution.
    • Security Scan 33502572716 → dependency-review 99839068988, osv-scan 99839069511, scorecard 99839069518, trivy-fs 99839069595: all queued/no steps. These are organization-required workflow jobs; ConceptWeave has no local Security Scan workflow to patch.
    • SAST Semgrep 33502572814 → Semgrep 99839069892: queued/no steps. ConceptWeave has no local SAST workflow to patch.

The repository-owned publication/evidence encapsulation findings are source-addressed on this head; there is still no executed exact-head Product failure from which to justify additional leaf source churn. The central owner path .github#712 has been refreshed with these exact coordinates and linked to the broader queue-health path (#1150/current successor). Existing central evidence has already falsified a blanket Actions-disablement explanation and an enforcing Actions alert budget; deeper selective hosted-runner scheduling/payment-account/provider capacity remains to be distinguished by the central owner or GitHub billing/support surface.

Do not cancel or duplicate-rerun these sole current-head lanes merely to create activity. Acceptance remains real runner assignment/execution on the unchanged exact head (with actual checkout SHA), or a deterministic exact external prerequisite from the central control plane, followed by fresh ConceptWeave revalidation.

Current-head refinement: PR #1 is now 971ddcbd27328e9f6d53c61bb16f7d5a05c97435. The additional leaf change replaces deprecated Node-20-era actions/checkout v4 with the same immutable actions/checkout@9c091bb21b7c1c1d1991bb908d89e4e9dddfe3e0 (v7.0.0) already used by protected central workflows; the predecessor executable log explicitly warned that checkout v4 targeted deprecated Node 20.

The Product concurrency repair is now empirically proven: run 33509921965 for predecessor 6520478… was automatically cancelled at 2026-09-01T12:54:08Z when 971ddcbd… synchronized. Central SAST run 33509921992 and Security run 33509922026 for that same predecessor were also cancelled by their central concurrency contracts. This removes three provably obsolete queue lanes without cancelling the sole current-head evidence.

Current sole Product evidence is run 33510237873, job 99863874307, exact head 971ddcbd…, ordinary ubuntu-latest, still pre-checkout/runner-unassigned at the latest read. Current Security 33510237886 and SAST 33510237814 are likewise non-passing while queued. Keep this head stable unless an executed current-head failure produces a real source repair.

Fresh control-plane refinement, 2026-09-01: the runner-admission RCA is now selector-specific rather than generic saturation.

    • ConceptWeave feat: establish ConceptWeave foundation #1 unchanged head 5cd7d1de742fe34aa99900641cc8b124e7c65f9e remains Product GREEN.
    • Its organization-required Security/SAST jobs still request ubuntu-latest, remain pre-execution, and expose runner_id=0/steps=[].
    • Central .github#1588 provided a same-repository control: head a502dc5d4e909e2e088947de038e0d3840d4d76c, Strix Changed Path Quality CI 33521385880 / job 99901247530 requested explicit ubuntu-24.04, acquired hosted runner 1001591835, completed exact checkout/setup, and executed tests; the same head's central Security Scan 33521385765 jobs requested ubuntu-latest and remained queued without runner assignment.
    • .github#712 has been updated with these exact ConceptWeave and central canary coordinates. The existing CWL .github Commercial Loop was also tightened to make explicit supported-image migration from the starving ubuntu-latest surface its immediate P0, using .github#1588 or a live successor rather than creating a competing writer.

ConceptWeave-side acceptance remains unchanged: do not move the clean source head to retrigger. After the central selector repair reaches protected truth, rerun/revalidate the unchanged current ConceptWeave head and require actual checkout SHAs for Security/SAST. If explicit ubuntu-24.04 later also stops acquiring runners, reopen billing/quota/org-policy/platform capacity as the primary competing cause.

Fresh runner-admission recurrence while advancing Client P0, not a source blocker conclusion: PR #5 test-only head de083fba1a257ed66935c9ff52963c8f397e7783 (adds only the resolve_concept RED) has Product run 33545538986, job 99981981775 queued with steps=null at repeated reads. Parent and predecessor PR #5 heads have acquired Ubuntu 24.04 hosted runners and exact checkout, so this is not enough evidence for an invalid leaf selector. Exact canary was added to central .github#712; do not mutate/retrigger the source merely for admission. This issue remains open for both queue-health recurrence and the separate foundation Security/dependency-review 403 tracked by .github#810.

    Current-head correction — 2026-09-02

The Client lane has advanced since the issue body snapshot. PR #5 test-only head 827080d101b3d057b4626d845f51d7d575f773f1 did acquire Ubuntu 24.04 runner job 100002392979 in Product run 33551686430; exact checkout and fmt succeeded, then Clippy produced the intended TDD RED with five E0599 failures because SemanticReleaseClient::resolve_concept was absent. The smallest deterministic implementation was then pushed non-destructively on the same writer branch; current head is 4b4c47f363f46ad6d2923ea076e12b34cbf15eaf. It validates nonblank input, reuses authoritative-release admission, and performs exact identifier lookup only—no fuzzy/case-folded match, model/provider call, source access, or authority promotion. Fresh Product run 33558403542 / job 100024764227 is currently queued with no executed steps, so GREEN is not yet claimed.

A separate Issue #2 Source Observation lane now exists as stacked Draft PR #6. Its current exact head is 4c1cfa23ae168394f2b1629217cfe56b274cc7ac; test-first lineage starts at c9b98dec13631d72a3616e99e73d59ced2ed0559, followed by the immutable Rust PostgreSQL schema-observation contract and code-current PRD/architecture/changelog/product-gap baseline. Product run 33558198185 / job 100024091488 is currently queued with no executed steps. No live database adapter or production database access is claimed.

Foundation PR #1 control-plane ownership is unchanged: Product and SAST are terminal success; Security run 33527150445 still has sibling OSV/Scorecard/Trivy success while dependency-review job 100008469220 remains queued/pre-checkout. .github#712, .github#810, and .github#772 remain the causal runner, Dependency Review availability, and solo-maintainer approval owners respectively. No leaf bypass or substitute-scanner promotion was used.

Fresh 2026-09-02 owner-state reconciliation after a material central queue-pressure change.

Central protected .github/main has advanced to exact 29b931e139ba12319de98f629cdae58479574bc5 through .github#1673 (refactor(actions): consolidate hourly review-repair callers). The integrated change collapses 18 scheduled review-repair callers into one reviewed scheduler and retires their persistent workflow identities. This is a real control-plane causal change, not a status-only comment, so ConceptWeave revalidated its preserved exact heads without source churn.

Current unchanged ConceptWeave admission evidence remains non-passing:

    • Client PR feat(client): add offline semantic release admission contract #5 exact head 398d8d6878706251397267d8fd8a660d4c2ecc7e: Product run 33584293306, job 100105057506, ubuntu-24.04, runner_id=0, no runner identity, steps=[].
    • Generation PR feat(observation): define immutable PostgreSQL schema snapshot contract #6 exact head 4e961c1ad221e0b0b71ae113485bddf42be8e561: Product run 33584395183, job 100105358688, ubuntu-24.04, runner_id=0, no runner identity, steps=[].
    • Foundation PR feat: establish ConceptWeave foundation #1 exact head bba351b77bf5f1ab5cfd55979fbb2bd158f78b81: Security attempt 3 is now blocked only at dependency-review job 100059571813, ubuntu-24.04, runner_id=0, steps=[]; sibling Trivy, Scorecard and OSV jobs on the same attempt acquired GitHub-hosted runners and completed successfully. The earlier HTTP-403 Dependency Review evidence remains historical for the previous executed attempt and does not substitute for this queued attempt.

A bounded unchanged-head rerun was attempted for current PR #5 job 100105057506 after the central causal change. GitHub rejected it with HTTP 403, The workflow run containing this job is already running. The current exposed rerun path therefore cannot duplicate an already-active queued run. No cancellation of the sole current-head evidence, no no-op commit and no source retrigger was performed.

The fleet incident is not yet closed: .github#712 remains open and its latest fresh evidence after protected .github/main@29b931e1… still shows multiple same-repository exact-head workflows pending/queued. Current classification is therefore intermittent/saturated hosted-runner admission with lane-level scheduling differences, not a ConceptWeave runs-on defect. GREEN remains actual runner assignment + exact checkout on these unchanged heads after the central queue-pressure repair drains, or a more precise central/admin capacity-policy prerequisite evidenced by the .github owner path.

GRC reference-client state was also re-fetched: governance-risk-compliance#63 remains open; stacked GRC PR #64 remains Draft/open/mergeable at exact ecfa729a29dfba6cd3c78046cda2d9d2518fe06e on Analytics root dbe7f5d42db0bb1b24d8a0b51eae5032d098d7f5. It still intentionally defers a concrete ConceptWeave adapter until a supported generic release/client contract exists.

Fresh 2026-09-02 runner/control-plane refresh; this supersedes the current-head coordinates in the issue body where they differ.

    • Client PR feat(client): add offline semantic release admission contract #5 exact head is 398d8d6878706251397267d8fd8a660d4c2ecc7e; Product run 33584293306, rust-quality job 100105057506, label [ubuntu-24.04], runner_id=0, steps=[], no checkout. The detached-artifact missing-method RED is still not observed, so production code remains withheld.
    • Generation PR feat(observation): define immutable PostgreSQL schema snapshot contract #6 exact head is bc25e6f29f16bf5afa8df220f466e688f030aa3e; Product run 33594432505, rust-quality job 100134904991, label [ubuntu-24.04], runner_id=0, steps=[], no checkout. The canonical sha256: validation RED is still not observed, so production validation remains withheld.
    • Foundation PR feat: establish ConceptWeave foundation #1 remains exact bba351b77bf5f1ab5cfd55979fbb2bd158f78b81; Security run 33527150445 is now attempt 3. Trivy, Scorecard and OSV are terminal success on that attempt, while dependency-review job 100059571813 is queued on [ubuntu-24.04] with runner_id=0, steps=[].
    • Central .github minimal canary PR #1652 is closed/unmerged, but canary run 33571492334 / job 100066209397 at dd2c20bccefa604f972caf61900a26f942cc80a0 remains queued with runner_id=0, steps=[] despite having no checkout/action/matrix/needs/environment/credential and only one run: true step.
    • Fresh .github queued-run inventory reports 1593 queued runs. The existing CWL .github Commercial Loop has been advanced, not duplicated, to test-first repair two current pressure amplifiers: runner-backed close-event no-op cleanup jobs (using #1588 as the proven pattern) and runner allocation on COMMENTED-only pull_request_review scheduler events. Exact evidence is recorded on .github#712.

No ConceptWeave source head is being moved merely to retrigger admission. After a causal central repair reaches protected .github/main, preserve the then-current #1/#5/#6 heads and require actual runner assignment plus exact checkout before continuing their respective evidence sequences.

2026-09-02 live child-head refresh: Client PR #5 has advanced to exact head 9688dde08368a9324d64abe25122036719dcc1fd; Product run 33610145373 is queued before execution and therefore non-passing. Generation PR #6 remains e56054cdce716a91759294b1993b31a1ca93ed57 with Product 33609118662 queued. Foundation PR #1 remains unchanged at bba351b77bf5f1ab5cfd55979fbb2bd158f78b81 with Product/SAST terminal success and Security fail-closed at Dependency Review availability. The live organization ruleset still requires one approving review with no declared required reviewers plus required central workflows/thread resolution. Product run 33610537831, job 100184362749, is still queued on ubuntu-24.04 with runner_id=0, empty runner identity, and steps=[]. This is the intentional test-only language-neutral supersession/publication-contract head; do not advance production until its missing-contract RED executes.
    • Generation PR feat(observation): define immutable PostgreSQL schema snapshot contract #6 is now Draft/open/mergeable at exact f374476affe4753287366628c55c9b3af1a2db5c. Product run 33632391908, job 100254801402, is still queued on ubuntu-24.04 with runner_id=0, empty runner identity, and steps=[]. • Two consecutive fresh reads returned the same feat(client): add offline semantic release admission contract #5/feat(observation): define immutable PostgreSQL schema snapshot contract #6 pre-execution state. No no-op push, cancellation, duplicate rerun, runner-label substitution, predecessor-evidence transfer, or gate weakening was performed.
    • Central owner .github#712 already contains the exact current ConceptWeave handoff and remains the queue/admission repair owner. .github#810 remains the Dependency Review availability owner. Current organization ruleset 18156473 still requires one approval and the central required workflows; ConceptWeave does not self-approve or bypass it.
    • ConceptWeave still has no release. Release remains prohibited until one integrated protected head has fresh terminal quality/security/review/package/SBOM/provenance/compatibility evidence.

Next executable leaf action remains evidence-driven: if either Product job acquires a runner, inspect the first actual RED and repair only that causal contract on the existing branch. Until then, preserve these exact test-only heads and continue central owner-path RCA rather than manufacturing new source evidence.

Fresh owner update — 2026-09-02 UTC. The prior #5 queued coordinate is superseded by an actual runner-backed failure and a causal leaf repair.

    • Client feat(client): add offline semantic release admission contract #5 test-only 143506eb66b6904b770a628ac793af2253559df2 finally acquired hosted Ubuntu 24.04 runner 1001622251. Product 33610537831 / job 100184362749 verified exact checkout and failed first at cargo fmt --all --check; Clippy/tests/contracts never ran. The job log supplied an exact rustfmt diff in three Client files.
    • Dedicated owner applied only that rustfmt output as fast-forward commit 0c32a7b55d3c687ab76cee789962866573496ba1. No missing supersession schema/fixture or semantic behavior was added. Current Product 33664177838 / job 100361706615 is queued pre-execution on ubuntu-24.04; this remains the sole head that may establish the intended missing-contract RED.
    • Generation feat(observation): define immutable PostgreSQL schema snapshot contract #6 remains unchanged at f374476affe4753287366628c55c9b3af1a2db5c; Product 33632391908 / job 100254801402 is still queued pre-execution with runner_id=0, steps=[], explicit ubuntu-24.04. Do not churn it before the UTC provenance RED executes.
    • Fresh .github/main@8c085835fbf77de2321b72fa6b8dd946227e523e still admits every pull_request_review: submitted event into runner-backed scan-pr-queue without excluding COMMENTED state. Exact current feat(client): add offline semantic release admission contract #5/feat(observation): define immutable PostgreSQL schema snapshot contract #6 evidence and this queue-amplification RED were handed to canonical .github#712 as comment 5513902307; central source remains .github-writer-owned.
    • Fresh GRC coordinates remain develop@85da4e1c64e5b46ca3a11ca71654a82b57366a3e, Issue #63 open, reference-client PR #64 Draft/open/mergeable at ecfa729a29dfba6cd3c78046cda2d9d2518fe06e on root dbe7f5d42db0bb1b24d8a0b51eae5032d098d7f5.
    • ConceptWeave still has zero GitHub releases. No Draft/open head is supplier authority.

Acceptance remains fail-closed: #5 must execute past fmt and show the intended contract RED before production implementation; #6 must execute its timestamp RED before production validation; central queue/dependency-review/governance owners must repair their own boundaries without leaf bypass or predecessor evidence.

Additional current-head RCA for #35: Strix failure 33938445050 is independently explained by gateway/provider failover rather than the one-file ConceptWeave workflow diff. Job 101256562088 passed admission, target materialization, sidecar provisioning and gateway preflight; artifact strix-reports id 9969701340 (sha256:689e780747d5a2d7b1f5857f7d2a8900cd166b6694e6fa3a324c49c2e53ccf06) records 62 admitted free routes / 3-account diversity / 12 selected / 3 ready, followed by repeated 500 internal_error responses during the real Strix session and fail-closed STRIX_PROVIDER_UNAVAILABLE after ~7,014 s. No vulnerability verdict was produced. Canonical gateway repair/evidence owner is contextual-orchestrator#1045, now carrying this specimen in comment 5552624035. 2026-09-06 authority delta — this comment supersedes only the stale OpenCode/#6 coordinates in the issue body; all other owner boundaries remain unchanged.

    • Product bootstrap chore(ci): bootstrap Product pull-request workflow #35 remains a31ae0c2df920f2794f7ddb456795b04797ab472, open/non-Draft/mergeable. Required OpenCode run 33938445067 is no longer queued: job 101324750468 completed failure after validating the unchanged live head, obtaining OIDC and the repository-scoped app token, and successfully POSTing the central opencode-review dispatch. Its immediate receipt gate then failed closed because no authenticated current-head APPROVED/CHANGES_REQUESTED verdict existed yet.
    • The canonical owner execution exists as .github run 33977685099, OpenCode Review Dispatch ContextualWisdomLab/ConceptWeave#35@a31ae0c2df920f2794f7ddb456795b04797ab472, created by opencode-agent[bot] on central main@7f4c5e3e0efb7bfe29f33b60d4264858effd2996; fresh read still shows queued. Therefore the leaf failure is not evidence of OIDC/token/dispatch failure and is not a reason to cancel/replay the central handle or manually rerun the unchanged leaf wrapper.
    • Source Observation feat(observation): define immutable PostgreSQL schema snapshot contract #6 has advanced normally to 42c94c9b5260b936a90fe2e225500dc32c253889, still Draft/open/mergeable. Static compile-contract RED b2b83c0fdc78af11e3e0df8cf6993216dd9c6004 specifies an awaitable provider-independent Send source-port future; production 638be096f444fd22755160972285dbb9f0eb0364 makes only authorized live adapter execution awaitable while keeping request admission/registry authorization deterministic and preserving one operation deadline. Existing cancellation/zero-side-effect fixtures and ADR/TRD/Architecture/CHANGELOG were adapted. No Rust toolchain or repository-owned PR Product run exists for this exact head, so this is source-repaired/verification-pending, not GREEN.

The Zotero/full-text/write-admission single-writer lane remains out of scope. No #35 source/head change, blind rerun, review dismissal, approval, protected merge, or release is claimed.

Fresh repository-owned coordinate update: Source Observation #6 is exact 77c143d4ef8a6d8d23196682d6e3b0fd954cd115, not eafc587.... The one-commit successor changes only the active product/technical gap baseline and has no hosted workflow generation; historical Rust/coverage evidence remains non-transferable. The representation P0 now explicitly includes review 5160252371: columns must bind exact qualified PostgreSQL type evidence rather than the current unqualified data_type: String, with enum label/order and material domain semantics included in successor identity. Review 5160201671 still governs backward-compatible schema-scoped receipts and type-only-schema authorization. Exact-head review 5160292623 records the docs-only/non-GREEN boundary. Central owner coordinates remain .github/main@7fd571db... and #2040 6706c231...; no leaf rerun is justified.

Current prerequisite correction — .github#2350@7a627f3e9c564bcf7ce2f826b57c1b6ef8d1ebae remains the Product protected-policy owner. Its predecessor immutable-coordinate contract RED has executed, but exact-current review 5289945740 found the repair is incomplete: product_workflow_blob_sha is parsed/comparable yet not threaded into bootstrap/canary/activation, so null does not yet fail closed and marker-compatible protected Product drift could still reach evaluate/active mutation. Owner run 35845521134 remains runnerless queued, so no source-neutral wake or subsequent source head is justified. Required order remains #2350 exact-head settlement → causal blob-bound mutation repair → supported-event canary repair 5288830215 → #35 terminal evidence/normal landing → protected Product blob adoption → evaluate ruleset/real Foundation synchronize canary → guarded active promotion → Foundation/semantic propagation.

Fresh Source Observation update: #46 advanced ordinary-forward from e35de25355cb4aa4bc75d5900a6e09803d80ff41 to 4244f5802708124cdc0d64137820712edbd68d87 (1 commit, 1 file, +10/-0). This is still the same 5293650822 schema-wide pg_class namespace RED, not a new semantic finding: the retained contract now also covers the same-relation edge where public.document owns an index named document. Review 5295345693 pins the minimal eventual production boundary to canonicalize_snapshot_objects() before digest/receipt construction: one schema-local namespace over modeled relation names plus child index names, preserving relation-scoped index ownership and allowing equal names across schemas.

No GREEN is transferred or claimed. Fresh exact-head workflow inventory for 4244f580... is still empty, so namespace and existing semantic threads remain execution-gated. Parent #45/#6/#5 must not partially adopt this child delta. Owner-plane state is otherwise unchanged at this read: #2350 run 35891410528 remains queued on a1628e75...; #35 downstream CodeQL dispatch 35870670165 remains queued; protected ConceptWeave main is f4f440dd58c77d7cd90dff8a1eb2eeb9a9940425, protected .github/main is e6334e229581a918e2f22de18733b76fa65d7e71.

2026-09-24 KST fresh correction to the issue body's stale coordinates (no leaf source mutation): Source Observation authority is now #46 exact 4244f5802708124cdc0d64137820712edbd68d87, not e35de253.... The extra commit is test-only and completes the existing pg_class namespace RED with the same-relation self-collision witness; review 5295345693 keeps the eventual aggregate guard child-owned. #46 still has zero PR-triggered workflow runs. Separately, #35 downstream .github CodeQL run 35870670165 has progressed: validate-dispatch 107213600319 is terminal SUCCESS; scan jobs 107322443235 (python) and 107322443261 (actions) remain queued. Therefore the CodeQL blocker is now scan admission/execution plus the already-terminal required-run reconciliation, not dispatch validation. #2350 owner run 35891410528 remains queued, so 5294002721 is still not staged.

Fresh owner-path correction — queue/admission continuation moved to .github#2356; no ConceptWeave source change.

Issue #712 remains the historical queue RCA owner but its discussion is comment-cap exhausted. .github#2356 is now the bounded writable continuation. Fresh ConceptWeave specimens are recorded there in comment 5805398442:

    • .github#2350@a1af52d7bf2fcb5dfd27790961faf62651b16510, owner run 35918073452, validate 107374677579: [ubuntu-24.04], queued, runner_id=0, steps=[], current/irreplaceable exact owner evidence;
    • chore(ci): bootstrap Product pull-request workflow #35 downstream run 35870670165, settlement 107396154783: [ubuntu-24.04], queued, runner_id=0, steps=[], current downstream settlement.

The same #35 downstream run is not uniformly queue-starved: validate-dispatch is SUCCESS and both language scans obtained runners, then failed only at the central GHAS configuration-identity proof after analysis/SARIF gates succeeded. Keep those failures with .github#1929/#2275/#2276; keep new queue-health evidence with #2356 (historical context still #712).

Required sequencing is unchanged: preserve #2350 a1af52d... until exact owner terminal evidence; only GREEN permits 5294002721 RED/minimal final-ref repair/GREEN. Do not move #35/#46 or create a no-op wake for central evidence.

Owner-plane currentization: .github#2350 predecessor a1af52d7bf2fcb5dfd27790961faf62651b16510 has exact Product Ruleset Reconcile GREEN (35918073452 SUCCESS). Current ordinary-forward test-only authority is 1a6ddce2d02df50dddeb81450ff05cb9778c94a9, implementing review 5294002721 as a reality-RED candidate: if protected .github/main or ConceptWeave main advances during the final exact Product blob read, bootstrap evaluate creation / activation PUT must not occur. Production reconciler bytes remain unchanged. Current owner run 35944402217 / validate 107459043420 is pre-runner queued, so no production fix or GREEN is claimed yet. #35 remains externally blocked at central GHAS configuration identity (.github#1929/#2275/#2276); its Python/Actions scans are terminal at that exact verification step, with settlement still queued. Source Observation #46 remains independent and must retain its existing pg_class namespace RED until native Product/Rust/PostgreSQL execution.

    Fresh authority correction (2026-09-24 KST):

    • chore(ci): bootstrap Product pull-request workflow #35 downstream .github run 35870670165 is now fully terminal. Settlement 107396154783 is FAILURE, not runnerless queued: it obtained GitHub-hosted runner 1002115554 and failed at Settle exact CodeQL required run. Reclassify it from queue/admission to central lifecycle/recovery evidence. The Python/Actions shards remain terminal GHAS-identity failures after analysis + Medium+ SARIF success, owned by .github#2275/#2276; settlement's exact failing guard/token cannot be asserted without step stderr.
    • .github#2350 has advanced after exact GREEN on a1af52d... to 1a6ddce2d02df50dddeb81450ff05cb9778c94a9, the test-only reality-RED head for review 5294002721. Owner run 35944402217 remains queued, so production final-ref revalidation must not be applied yet.
    • Queue continuation authority is .github#2356 (the writable successor to comment-capped #712). #2356 now records the settlement's same-head queue→runner transition and retains #2350@1a6ddce.../35944402217 as the current ConceptWeave queue specimen.

No ConceptWeave semantic source head needs to move from this update. Current causal order remains #2350 1a6ddce hosted RED -> minimal final protected-ref repair -> GREEN -> separate supported-synchronize/Open-Draft canary repair -> #2350 source landing without live mutation -> #35 central evidence/landing -> protected Product blob adoption/evaluate -> Foundation canary/active proof -> semantic-stack propagation.

Fresh owner-plane coordination update (2026-09-24 KST): .github#2350@1a6ddce2d02df50dddeb81450ff05cb9778c94a9 is still preserved for review 5294002721. Commit-level check inventory now shows 14 checks on that exact generation: 13 remain queued and only cancel-closed-pr-runs 107459042544 is completed/skipped. Queued checks span Product validate 107459043420, multiple independent Detect changed scope jobs and gitleaks, so the current wait is not specific to the Product reconciler or its PR-number concurrency key. Canonical queue-control evidence is recorded in .github#2356 comment 5807249330; do not wake the leaf/source, blind-rerun, inflate timeouts, or weaken concurrency/gates.

A separate semantic-stack coordination finding is now explicit on #43: its named Foundation branch has advanced to 84e56256a794b11568c271818c55c899ecfcbb60, while #43 exact 2c6d3037acdeae2b152c335ee2f60a59b4472831 compares as diverged 5-ahead/2-behind with merge base 60f14a6e85a83d56c2eea43b34d52b3366bb1735 and GitHub reports it non-mergeable. This is a retained stale-parent/conflict repair finding, not a close/rebase reason. Foundation remains prerequisite/acceptance-pending; after its complete accepted reconciliation, #43 must ordinary/non-force adopt the whole parent and then #44 restacks. No partial semantic/Product fix is copied downstream.

Product-governance prerequisite advanced without ConceptWeave source movement.

.github#2350@1a6ddce2d02df50dddeb81450ff05cb9778c94a9 is now attributable hosted RED: run 35944402217, validate 107459043420 acquired runner 1002117594; exact checkout/tooling/manifest validation succeeded, lifecycle-contract proof failed, and mutation/live jobs were skipped. This closes the queue-only uncertainty around review 5294002721.

Minimal repair is ordinary-forward .github#2350@97acdfeef0830d56c7b9b78226fec53de791ee8f: after the final reviewed Product blob read, bootstrap and activation re-read protected .github/main and ConceptWeave main immediately before evaluate POST / active PUT. Commit diff is only those four executable lines. Exact owner run 35961585829 / validate 107511123583 is currently queued; do not wake or rerun it blindly. Product-governance owner update (2026-09-24 KST): .github#2350@97acdfeef0830d56c7b9b78226fec53de791ee8f did receive a hosted runner and failed only in the lifecycle unit-contract step; exact checkout/tooling/manifest validation passed and live mutation/verification remained skipped. RCA is the retained test_bootstrap_create call-count expectation after the intentional post-blob protected-ref revalidation was added. The minimum test-only ordinary-forward fix is .github#2350@04414c5e30ba6aed2bf3d82268b356c6d6b1ad84, changing only the expected .github/main and ConceptWeave-main revalidation counts. New owner run 35983462019 / validate 107580552666 is currently queued pre-runner. Docs-to-code gap surfaced in fresh review: Foundation exact 84e56256a794b11568c271818c55c899ecfcbb60 still carries docs/product-technical-gap-baseline.md with snapshot 2026-09-09; it names historical #35 22709ec... and central .github#2051@70e8c1f..., so it is no longer a code-current operational-state baseline.

Do not repair this by a docs-only wake commit while .github#2350@04414c5... is preserving irreplaceable exact owner evidence and Foundation is still the canonical parent for multiple Draft stacks. That would reset Foundation exact-head evidence and widen stale-parent propagation without closing the Product prerequisite. Treat the file's product-gap list as useful design content but its protected/PR/run coordinates as historical until normal Foundation reconciliation.

Required insertion into the existing order: after Product owner/source prerequisites and #35 normal landing, ordinary/non-force reconcile Foundation and in that same substantive head refresh docs/product-technical-gap-baseline.md (plus PRD/TRD/ADR/CHANGELOG where code-currentness changed) against live protected state before propagating to #9 and #5. This is a tracked docs-to-code repair, not acceptance evidence and not permission to move Foundation early.
