- List page: remove "Playtest Hub" title header, keep tabs only
- Create form: fix bug distribution_model lost on step navigation (use getFieldsValue(true)); remove page header; switch action bar to sticky to stay within AGS extend app boundary
- Detail page: replace breadcrumb with "Back to list" link + arrow; add subtle top border to header
- Distribution tab: replace OK build-health alert with green badge + tooltip showing last-checked time; combine info alerts into single blue info alert with 2 bullet points
