Issue loading as an AMD module

dependencies are not specified and this is causing errors when loading using RequireJS

I am not sure how to specify dependencies?
define(['exports','knockout'], factory);
or
define(['exports','knockoutjs'], factory);
or something else

I got it to work locally. I'll give you a pull request so you can test it
in a non AMD environment. It should work.
