Steps to Reproduce

1. Create a custom LLM Provider Template and create a new version of the same (say v1 and v2)
2. Create 2 LLM Providers using the 2 template versions
3. There is no way to determine what template version was used to create each provider by just looking at the UI. However this information is available within the REST API GET under the template field.
