Guardrails

• Local-first only
• No SaaS, billing, SSO, or hosted service
• Trusted packages cannot bypass hard blocks
• Expired exceptions fail
• Override reasons are not leaked unsafely
• Behavior analysis remains disabled by default

Summary:

• Added policy schema versioning and validation for schema_version: "1.0".
• Added hard-block invariant checks so critical malware, credential, dependency-confusion, private-registry, and shell download/execute controls cannot be disabled or weakened.
• Added exception audit validation and force-risk reason enforcement.
• Added pkgsafe policy test [--json] <file-or-dir> with enterprise policy fixtures.
• Enhanced policy explain with schema, override audit, and hard-block status.

Validation passed: gofmt -w ., go test ./..., go test -race ./..., go vet ./..., make build, make package, policy validate/explain/test, YAML parse, markdown links, wording audit.

Known caveat: this branch is stacked on uncommitted Loop 1-4 work.
